چکیده

بیماری مزمن کلیه (CKD) یکی از مشکلات اصلی نظام‌های سلامت در سطح جهان محسوب می‌شود و با اختلال در تعادل میکروبی روده (دیس‌بیوز) ارتباط تنگاتنگی دارد. دیس‌بیوز با تولید سموم اورمیک و تشدید التهاب، به پیشرفت نارسایی کلیوی کمک می‌کند. این مرور روایتی با هدف بررسی شواهد جدید منتشرشده بین سال‌های ۲۰۲۰ تا ۲۰۲۵ درباره تأثیر پروبیوتیک‌ها و سین‌بیوتیک‌ها بر تنظیم محور روده-کلیه و بهبود پیامدهای بالینی در بیماران مبتلا به CKD انجام شده است. ابتدا، با جستجوی ساختاری در پایگاه‌های PubMed ،Scopus‌ و‌ ScienceDirect ۱۵ مطالعه بر اساس معیارهای تعیین شده انتخاب و نتایج آنها تحلیل شد. بررسی مطالعات انجام شده نشان داد که این مداخلات قادر به بازسازی مطلوب ترکیب و عملکرد میکروبیوتای روده هستند؛ این امر از طریق تقویت جمعیت باکتری‌های مفید تولیدکننده اسیدهای چرب کوتاه‌زنجیره (SCFAs) و کاهش حضور عوامل بیماری‌زا صورت می‌گیرد. این تغییرات مثبت میکروبی، با کاهش چشمگیر سطوح سموم اورمیک (نظیر ایندوکسیل سولفات و پی-کرزیل سولفات) و نشانگرهای التهابی سیستمیک (از جمله اینترلوکین6 و کالپروتکتین) همراه بوده و همچنین بهبود قابل توجهی در علائم گوارشی و کیفیت زندگی بیماران گزارش شده است. شواهد تکمیلی حاصل از فناوری‌های پیشرفته اومیکس و نتایج تصادفی‌سازی مندلی نیز رابطه سببی بین میکروبیوتای روده و ایجاد CKD را تأیید می‌نمایند. نتایج این مرور، پتانسیل بالای بهره‌گیری از پروبیوتیک‌ها و سین‌بیوتیک‌ها در تعدیل محور روده-کلیه و مدیریت بهتر CKD را نشان می‌دهد. هرچند وجود ناهمگونی در نتایج، لزوم انجام پژوهش‌های آینده طولانی‌مدت‌تر و یکپارچه را ایجاب می‌کند تا شخصی‌سازی درمان بر اساس داده‌های اومیکس در آینده نزدیک امکان‌پذیر گردد.
