چکیده
درمان نوروپاتی محیطی چالشبرانگیز است؛ درک مکانیسم زیربنایی ممکن است منجر به انتخاب مؤثرترین درمانها شود. این مقاله مکانیسمهای زیربنایی رویکردهای فیزیکی برای درمان انواع مختلف نوروپاتی محیطی را بررسی میکند. پایگاههای داده PubMed، ScienceDirect و Google Scholar جستجو شدند. پس از غربالگری 161 مقاله و حذف موارد نامرتبط، 36 مطالعه شامل کارآزماییهای بالینی تصادفی و غیر تصادفی و همچنین بررسیهای سیستماتیک انتخاب شدند. برای تسهیل تجزیه و تحلیل، مطالعات بر اساس شرایط زمینهای به سه گروه طبقهبندی شدند: (1) نوروپاتی محیطی ناشی از شیمیدرمانی، (2) نوروپاتی محیطی دیابتی و (3) سایر انواع نوروپاتی محیطی. در گروه اول، مداخلات مختلف فیزیوتراپی و روشهای مختلف ورزشی بررسی شد، در گروه دوم اثرات روشهای مختلف ورزشی ارزیابی شد، و در گروه سوم ورزشهای هوازی و مقاومتی پیشرونده و همچنین درمان با ایجاد توهم لمس بررسی شد. این طبقهبندی امکان ارزیابی مداخلات توانبخشی مختلف مورد مطالعه در هر زیرگروه از نوروپاتی محیطی را ایجاد کرد. این مطالعه مروری نشان داد که مکانیسمهایی مانند محافظت عصبی، بازسازی عصب، اثرات ضد التهاب، کنترل قند خون، بهبود عملکرد عروقی، تعدیل درد، فعالسازی عضلات و نوروپلاستیسیته در اثربخشی این مداخلات نقش حیاتی دارند.
