چکیده
تصلب شرایین، یک بیماری التهابی مزمن شریانهای بزرگ است که اصلیترین عامل رویدادهای قلبیعروقی محسوب میشود. دیسبیوزیس به عدم تعادل در ترکیب میکروبهای روده اطلاق میشود. این نوشتار یک مرور روایتی با جستجوی پایگاههای اطلاعاتی PubMed، Scopus،Google Scholar و Embase و با استفاده از مقالات و متون علمی منتشرشده طی سالهای 2000 تا 2025 تدوین شده است. مطالعات نشان میدهند که دیسبیوزیس میکروبیوتای روده از طریق تنظیم مسیرهای متابولیک، ایمنی و التهابی در تصلب شرایین نقش دارد. دیسبیوزیس با افزایش نفوذپذیری روده و انتقال لیپوپلیساکاریدها به گردش خون، به التهاب سیستمیک خفیف دامن میزند. همچنین، تولید اسیدهای چرب با زنجیره کوتاه کاهش مییابد و عملکرد لایه اندوتلیال عروق و سد مخاطی مختل میشود. در همین حال، تریمتیلآمین اِناُکسید (trimethylamine N-oxide (TMAO)) تولیدی توسط این باکتریها با فعالسازی مسیر فاکتور هستهای کاپا-بی (nuclear factor-kappa (NF-κB)) و اینفلامازوم (NOD-like receptor protein 3 inflammasome) Nlrp3، مهار انتقال معکوس کلسترول و سنتز اسیدهای صفراوی، ایجاد استرس شبکه اندوپلاسمی، تشکیل سلولهای فوم (foam cells) و افزایش واکنشپذیری پلاکتی، فرآیند تصلب شرایین را تشدید میکند. متعاقب دیسبیوزیس، سیگنالینگ اسیدهای صفراوی از طریق گیرنده فارنسوئید اِکس (farnesoid X receptor (FXR)) و (Takeda G protein-coupled receptor 5) TGR5 تغییر میکند و به تشدید التهاب عروقی منجر میشود. دادههای بالینی نشاندهنده الگوهای میکروبی متفاوت در مبتلایان به تصلب شرایین کرونری، و همچنین ارتباط بین دوز تریمتیلآمین اِناُکسید و پیامدهای نامطلوب قلبیعروقی هستند. مداخلات تغذیهای، پروبیوتیکها، پیوند میکروبی مدفوع و ورزش نویدبخش راهکارهای پیشگیرانه و درمانی جدید هستند.
