چکیده

مقدمه: توجه به حالات روان‌شناختی بیماران مبتلا به مالتیپل‌اسکلروزیس (MS) به‌دلیل تأثیر چشمگیر بر سیر و پیش‌آگهی بیماری بسیار مهم است. پژوهش حاضر به‌منظور مقایسه‌ی اثربخشی گشتالت‌درمانی، فعال‌سازی رفتاری و دارونما بر تنظیم شناختی هیجان بیماران مبتلا به مالتیپل‌اسکلروزیس انجام شد.
روش‌ها: روش پژوهش نیمه‌آزمایشی طرح پیش‌آزمون و پس‌آزمون با گروه کنترل و پیگیری سه ماهه بوده است و در آن شصت نفر از مبتلایان به مالتیپل‌اسکلروزیس مراجعهکننده به کلینیک MS بیمارستان رسول تهران انتخاب شدند. سپس با استفاده از بلوک‌های چهارتایی به‌صورت تصادفی در چهار گروه پانزده نفره، شامل سه گروه مداخله و یک گروه کنترل گماشته شدند. اعضای گروه‌های روان‌درمانی یک ‌بار در هفته به مدت هشت جلسه‌ی نود دقیقه‌ای تحت درمان قرار گرفتند. در گروه دارونما به مدت هشت هفته از کپسول‌های حاوی آرد برنج یک ‌بار در روز استفاده شد. هیچ مداخله‌ایی طی این مدت بر گروه گواه صورت نگرفت. ابزار پژوهش، پرسش‌نامه تنظیم شناختی هیجانی گارنفسکی و همکاران (2006) بود. برای تحلیل داده‌ها از SPSS نسخه‌ی 25 استفاده شد.
یافته‌ها: نتایج نشان داد اجرای گشتالت‌درمانی و فعال‌سازی رفتاری در مقایسه با دارونما و کنترل به افزایش میانگین نمرات راهبردهای سازش‌یافته تنظیم شناختی هیجان (001/0=p) و کاهش میانگین نمرات راهبردهای سازش‌نایافته (001/0=p) در مراحل پس‌آزمون و پیگیری منجر شده است. گشتالت‌درمانی در مقایسه با فعال‌سازی رفتاری میانگین نمرات راهبردهای سازش‌یافته (027/0=p) را در مراحل پس‌آزمون و پیگیری بیشتر افزایش داده است و نیز میانگین نمرات راهبردهای سازش‌نایافته (020/0=p)  را بیشتر کاهش داده است.
نتیجه‌گیری: گشتالت‌درمانی در مقایسه با فعال‌سازی رفتاری و دارونما بر بهبود تنظیم شناختی هیجان بیماران مبتلا به مالتیپل‌اسکلروزیس مؤثرتر است.

Comparison of the Effectiveness of Gestalt Therapy, Behavioral Activation and Placebo on the Cognitive Emotion Regulation in Multiple Sclerosis Patients: A Randomized Clinical Trial

Introduction: Psychological state of multiple sclerosis patients is very important due to its strong influence on incidence, progression, prognosis, and treatment of this disease. The present study compared the effectiveness of gestalt therapy, behavioral activation and placebo on cognitive emotion regulation in Multiple Sclerosis patients.
Methods: This study was a quasi-experimental pretest-posttest design with a control group and three months follow-up in which sixty patients with multiple sclerosis referred to the MS Clinic of Rasoul Hospital in Tehran were selected. Using four blocks, the authors randomly assigned participants to four groups of 15 people, including three intervention groups and a control group. Members of psychotherapy groups were treated once a week for eight 90-minute sessions. In the placebo group, capsules containing rice flour were used once a day for 8 weeks.  The people in the control group received no intervention during this period. The research instrument was Garnefski's (2006) Cognitive Emotion Regulation Questionnaire. SPSS (25) was used for data analysis.
Results: The results showed that gestalt therapy and behavioral activation in comparison with placebo and control groups increased the mean scores of adaptive cognitive emotion regulation strategies (p=0.001) and decreased the mean scores of non-adaptive strategies (p=0.001) in post-test and follow-up. Gestalt therapy, in comparison with behavioral activation, increased the mean scores of adaptive strategies (p=0.027) and also decreased the mean scores of non-adaptive strategies (p= 0.020) more significantly in post-test and follow-up.
Conclusion: Gestalt therapy is more effective than behavioral activation and placebo in improving cognitive emotion regulation in patients with multiple sclerosis.
