چکیده

مقدمه: پوسیدگی دندانی از شایع ترین بیماری های مزمن در جهان است. علاوه بر تست­های کلینیکی، تکنیک­های رادیوگرافی به ویژه تکنیک بایت­وینگ ابزار اصلی تشخیص دقیق پوسیدگی های بین دندانی هستند.
روش‌ها: در این مطالعه توصیفی- مقطعی، تعداد ۵۰ کلیشه رادیوگرافی بایت­وینگ (۵۷۶ سطح دندانی) با کیفیت استاندارد از یک کلینیک رادیولوژی تهیه شد. نمونه­ها به طور جداگانه جهت تعیین وجود یا عدم وجود پوسیدگی بین دندانی و عمق آن، توسط سه نفر از اعضای هیأت علمی به عنوان gold standard و ۳۰ دانشجوی سال­آخر دندان­پزشکی بررسی شدند. از مشاهده­گرها درخواست شد، وجود و عدم وجود و عمق پوسیدگی سطوح بین دندانی را با توجه به معیارهای تعیین شده در پرسش نامه علامت بزنند. داده ها پس از جمع­آوری توسط نرم­افزار آماری SPSS نسخه ۲۲ تجزیه و تحلیل شدند و میزان حساسیت، ویژگی، ارزش اخباری مثبت، ارزش اخباری منفی، مثبت کاذب و منفی کاذب محاسبه گردید.
یافته­ ها: میزان توافق (کاپا) اعضای هیأت علمی و دانشجویان در مورد وجود پوسیدگی برابر با ۰۰۷/۰ ±۶۱۰/۰ بود که از نظر آماری معنی­دار بود (۰۰۱/۰>P).  میزان حساسیت و ویژگی تشخیص پوسیدگی بین دندانی توسط دانشجویان به ترتیب ۳/۷۰ ٪ و ۹/۸۹ ٪ بود. Interobserver reliability بین تشخیص دانشجویان دارای مقدار ضریب توافقی (کاپا) ۰۰۷/۰ ± ۸۵۷/۰ بود.
نتیجه گیری: توانایی دانشجویان سال­آخر دانشکده­ی دندان­پزشکی رفسنجان در تشخیص وجود پوسیدگی و تعیین پوسیدگی های عمیق (نزدیک پالپ) عالی و در تعیین عمق پوسیدگی در سطح مینایی و رسیده به DEJ خوب بود. قابلیت اطمینان دانشجویان نیز در تشخیص پوسیدگی عالی بود.

An Evaluation of the Ability of the Senior Dental Students of Rafsanjan University of Medical Sciences in Detecting Proximal Caries in Digital Bitewing Radiography in the Academic Year of 2019-2020

Introduction: Dental caries is one of the most common chronic diseases in the world.  In addition to clinical examinations, the use of radiographic techniques, especially the bitewing technique, is the main tool for the accurate diagnosis of proximal caries.
Methods: In this cross-sectional study, 50 standard-quality Bitewing radiographs (576 dental surfaces) were obtained from a radiology clinic. The samples were examined separately by three faculty members as gold standard and 30 senior dental students to detect the presence or absence of dental caries and their depth. The observers were asked to mark the presence and absence and depth of dental caries according to the criteria specified in the questionnaire. The findings were analyzed by SPSS software Version 22 and the sensitivity, specificity, positive predictive value, negative predictive value, false positive, and false negative were calculated.
Results: The rate of agreement (Kappa) between the faculty members and students in detecting  caries was 0/610 ± 0/007 that was statistically significant (p < 0/001). The sensitivity and specificity of the students' diagnoses of proximal caries were 89/9% and 70/3%, respectively. Interobserver reliability among the students’ detection of caries yielded a Kappa coefficient of 0/857 ± 0/007.
Conclusion: The ability of senior  students of  Rafsanjan Dental School to diagnose the presence of caries and detect deep caries (near the pupl) was excellent, and their ability to detect caries on the enamel surface and caries reaching the DEJ was moderate/good. The students' reliability in diagnosing caries  was also excellent.
