چکیده

مقدمه: بیماری پیلونیدال ساکروکوکسیژیال یک بیماری نسبتاً شایع در بالغین است. این بیماری اغلب در مردان جوان و مناطق مدیترانه ای و اطراف خلیج فارس یافت می شود. روش های درمانی بسته به شدت و وسعت بیماری به کار می رود که شامل برداشتن ضایعه بدون ترمیم اولیه، همراه با بستن اولیه زخم و یا استفاده از پوشاندن زخم به وسیله فلاپهای مختلف می باشد.
روش ­ها: در این مطالعه مقطعی، 405 بیمار مبتلا به پیلونیدال ساکروکوکسیژیال بررسی شدند. این بیماران در بیمارستان نمازی و شهید فقیهی دانشگاه علوم پزشکی شیراز در بین سالهای 2006 تا 2011 تحت درمان جراحی قرار گرفتند. داده های دموگرافیک بیماران ثبت شده و روش های جراحی و عوارض تحت بررسی قرار گرفتند.
یافته‌ها: اغلب بیماران مردان جوان بودند (265 نفر، 4/65%) و بیشترین عارضه ترشح و درد در ناحیه ساکروکوکسیژال ذکر شد (2/87% و 353 نفر). مقایسه روش های جراحی شامل باز گذاشتن زخم، ترمیم اولیه بدون فشار و استفاده از فلپ رومبوئید نشان دادند که عود در تمام 3 نوع تکنیک پایین است (33 مورد 2/8%) به خصوص در جراحی با رومبوئید فلپ. غذاهای با پروتئین بالا و کافئین بالا (291 مورد 8/71%) و زندگی در محیط گرم و هوای مرطوب (348 مورد 9/85%) به عنوان فاکتورهای مستعد کننده در پیشرفت بیماری محسوب شدند.
نتیجه­ گیری: بر اساس نتایج اولیه، روش رومبوئید/لیمبرگ فلپ به عنوان جراحی پیشنهادی در درمان بیماری  پیلونیدال ساکروکوکسیژیال مطرح شد. بررسی های بیشتر برای یافتن نقش فاکتورهایی از قبیل غذاهای با پروتئین بالا و کافئین بالا بر بروز بیماری پایلونیدال نیاز است.

Introduction: Sacrococcygeal Pilonidal disease is a relatively common disease with high morbidity. This medical condition has been more detected in young adult men. Different types of management have been applied depending on the extension and severity of the disease, including resection without repair, primary closure, and using different methods of flap reconstruction.
Methods: In this cross-sectional study, 405 patients with the pilonidal disease were evaluated. Patients treated with different surgical techniques between 2006 to 2011 in Shahid Faghihi Hospital and Namazee Hospital of Shiraz University of Medical Sciences. Demographic data were registered and the surgical methods and complications were identified.
Results:Most of the patients were young men (265 cases, 65.4%) and their main complaints were discharge and pain (353 cases, 87.2%) in the Sacrococcygeal area. Comparison of the surgical methods including resection with an open wound, primary closure, and Rhomboid flap indicates that recurrence was seen low in all three techniques (33 cases, 8.2%), especially in the surgery using Rhomboid flap. High-protein and high-caffeine meal (291 cases, 71.8%) and living in the warm and humid climates (348 cases, 85.9%) have been evaluated as predisposing factors for disease development.
Conclusion:Due to primary results, the rhomboid/Limberg flap procedure should be considered as the preferable surgery method for the treatment of Sacrococcygeal Pilonidal disease. Further investigations are inevitable to find the role of factors like high-protein and high-caffeine meals on the incidence of pilonidal disease.
