نگرش و عملکرد بیماران دچار نارسایی احتقانی قلبی نسبت به مصرف خودسرانه دارو در بیماران بستری شده در مراکز آموزشی درمانی دانشگاه علوم پزشکی ارومیه
مقدمه: برای درمان نارسایی قلبی از یک رژیم دارویی پیچیده و متنوعی استفاده می شود. این در حالی است که بیماران داروهای دیگری علاوه بر داروهای تجویزی مصرف می کنند. مصرف غیرمنطقی و خودسرانه دارو علاوه بر عدم درمان بیماری باعث ایجاد عوارض دارویی در طولانی مدت و گاه وخامت حال بیماران می شود. لذا این مطالعه با هدف تعیین نگرش و عملکرد بیماران نارسایی احتقانی قلبی نسبت به مصرف خودسرانه دارو در بیماران بستری شده در مراکز آموزشی درمانی دانشگاه علوم پزشکی ارومیه انجام گردید. روش ها: روش نمونه گیری مطالعه از نوع تصادفی ساده بود. اطلاعات با استفاده از روش های آمار توصیفی در قالب جداول و آمار تحلیلی با استفاده از نرم افزار SPSSنسخه 18مورد تجزیه و تحلیل آماری قرار گرفت. لذا این مطالعه با هدف بررسی استفاده خودسرانه دارو در 200 نفر از بیماران نارسایی قلبی ارومیه انجام گردید. یافته ها: 98 درصد بیماران برای تجدید نسخه دارویی نیاز ی به مراجعه به پزشک احساس نمی کردند. 77 % بیماران برای رفع مشکل خود با توصیه دیگران دارو مصرف می کردند. 77 % افراد مورد مطالعه، داروهای بدون نسخه از داروخانه تهیه می کردند. نتیجه گیری: بی توجهی به مصرف منطقی دارو می تواند پیامد های نامناسبی چون نارضایتی بیمار، طولانی شدن و تشدید بیماری در بر داشته باشد. لذا اقداماتی از قبیل عدم فروش دارو بدون نسخه، افزایش اگاهی مردم در مورد استفاده از داروها و عوارض جانبی مصرف دارو قابل توصیه است.

تطبیق مقالات حاصل از مطالعات کیفی در مجلات پرستاری ایران از سال 2016 تا 2018 بر اساس گایدلاین COREQ
مقدمه: مطالعات کیفی در جهان از رشد روزافزونی برخوردار بوده اند و گایدلاینی که این مقالات را ارزیابی می کنند، چک لیست کورک می باشد. در مطالعاتی که انجام شده، تحقیقات کیفی در حوزه های غیر پرستاری یا پژوهشهای کمّی با گایدلاینهای استاندارد مقایسه شده اند، اما مطالعه ای که به بررسی تطبیقی مطالعات کیفی پرستاری با گایدلاین کورک بپردازند موجود نمی باشند. در همین راستا مطالعه حاضر پی ریزی شد. روشها: مطالعه حاضر از نوع توصیفی و بر روی مقالات کیفی سالهای 1395 تا 1397 انجام شد. برای جمع آوری اطلاعات از پرسشنامه اطلاعات دموگرافیکی و چک لیست کورک (32 آیتم) استفاده شد. داده ها با استفاده از نرم افزار SPSS ورژن 16 تجزیه و تحلیل شدند. یافته ها: نتایج حاکی از آن بودند که اکثر گویه ها در مقالات بررسی شده، با گایدلاین مطابقت داشتند. در مقایسه مقالات در مجلات با زبان انگلیسی و فارسی، در سه سال متفاوت، و تیپهای دانشگاهی متفاوت مشخص شد که رعایت برخی از گویه ها (10 و 15 و 18 و25 و28 و 29 و 30 و 31) و حیطه ها (حیطه 1) متفاوت بوده است. نتیجه گیری: رعایت نشدن برخی از گویه ها در مقالات کیفی منتشر شده در مجلات ایران این نکته را به سردبیران، نویسندگان و محققین خاطرنشان می سازد که بایستی با دقت بیشتری و بر اساس استانداردهای موجود عمل نمایند تا نتایج مطالعات از اعتبار و ارزش علمی جهانی برخوردار شود.

بررسی توانایی دانشجویان سال آخر دندان پزشکی دانشگاه علوم پزشکی رفسنجان در تشخیص پوسیدگی های بین دندانی بر اساس رادیوگرافی بایت وینگ دیجیتال در سال تحصیلی ۹۹-۱۳۹۸
مقدمه: پوسیدگی دندانی از شایع ترین بیماری های مزمن در جهان است. علاوه بر تستهای کلینیکی، تکنیکهای رادیوگرافی به ویژه تکنیک بایتوینگ ابزار اصلی تشخیص دقیق پوسیدگی های بین دندانی هستند. روشها: در این مطالعه توصیفی- مقطعی، تعداد ۵۰ کلیشه رادیوگرافی بایتوینگ (۵۷۶ سطح دندانی) با کیفیت استاندارد از یک کلینیک رادیولوژی تهیه شد. نمونهها به طور جداگانه جهت تعیین وجود یا عدم وجود پوسیدگی بین دندانی و عمق آن، توسط سه نفر از اعضای هیأت علمی به عنوان gold standard و ۳۰ دانشجوی سالآخر دندانپزشکی بررسی شدند. از مشاهدهگرها درخواست شد، وجود و عدم وجود و عمق پوسیدگی سطوح بین دندانی را با توجه به معیارهای تعیین شده در پرسش نامه علامت بزنند. داده ها پس از جمعآوری توسط نرمافزار آماری SPSS نسخه ۲۲ تجزیه و تحلیل شدند و میزان حساسیت، ویژگی، ارزش اخباری مثبت، ارزش اخباری منفی، مثبت کاذب و منفی کاذب محاسبه گردید. یافته ها: میزان توافق (کاپا) اعضای هیأت علمی و دانشجویان در مورد وجود پوسیدگی برابر با ۰۰۷/۰ ±۶۱۰/۰ بود که از نظر آماری معنیدار بود (۰۰۱/۰>P). میزان حساسیت و ویژگی تشخیص پوسیدگی بین دندانی توسط دانشجویان به ترتیب ۳/۷۰ ٪ و ۹/۸۹ ٪ بود. Interobserver reliability بین تشخیص دانشجویان دارای مقدار ضریب توافقی (کاپا) ۰۰۷/۰ ± ۸۵۷/۰ بود. نتیجه گیری: توانایی دانشجویان سالآخر دانشکدهی دندانپزشکی رفسنجان در تشخیص وجود پوسیدگی و تعیین پوسیدگی های عمیق (نزدیک پالپ) عالی و در تعیین عمق پوسیدگی در سطح مینایی و رسیده به DEJ خوب بود. قابلیت اطمینان دانشجویان نیز در تشخیص پوسیدگی عالی بود.

بررسی میزان آگاهی جراحان عمومی استان فارس از عوارض تنگی و ترومبوز وریدهای مرکزی به دنبال تعبیه کاتتر دبل لومن ساب کلاوین
مقدمه: احتمال ایجاد تنگی و ترومبوز در وریدهای مرکزی به دنبال تعبیه کاتتر دبل لومن در ورید ساب کلاوین نسبت به ورید ژوگولار داخلی بسیار بیشتر است. این مطالعه با هدف بررسی میزان آگاهی جراحان عمومی استان فارس از عوراض تنگی و ترومبوز وریدهای مرکزی به دنبال تعبیه کاتتر دبل لومن ساب کلاوین طراحی و انجام شده است. روش ها: این برررسی به صورت توصیفی-مقطعی بر روی جراحان عمومی استان فارس انجام شد و برای ارزیابی آگاهی آن ها از عوارض تنگی و ترومبوز متعاقب تعبیه کاتترمرکزی در ورید ساب کلاوین، یک پرسشنامه پژوهشگرساخته طراحی و به کار برده شد. یافته ها: در بین شرکت کنندگان، 63% از آن ها از ورید ژوگولارداخلی به عنوان روش ارجح تعبیه کاتتر دبل لومن استفاده میکردند درصورتی که 36% از آن ها روش ساب کلاوین را ترجیح میدادند. 46% از شرکت کنندگان آگاهی کافی در زمینه احتمال ایجاد تنگی و ترومبوزبه دنبال تعبیه کاتتر دبل لومن ساب کلاوین را داشتند و 4/47% احتمال ترومبوز را کمتر از 30% تصور میکردند. نتیجه گیری: نتایج به دست آمده از این مطالعه گویای آن است که درصد قابل توجهی از جراحان عمومی استان فارس نسبت به احتمال بالای ایجاد تنگی و ترومبوز در ورید ساب کلاوین به دنبال تعبیه کاتتر دبل لومن آگاهی کافی ندارند. لذا پیشنهاد میگردد جلسات آموزش مداوم و به روز برای جراحان عمومی در زمینه انتخاب ورید مناسب و همچنین استفاده از سونوگرافی جهت تعبیه کاتتر دبل لومن برگزار شود.

عوامل مرتبط در توانمند سازی مادران جهت شیردهی: مطالعه کیفی
مقدمه: تغذیه با شیر مادر برای شش ماه اول و ادامه تغذیه با شیر مادر تا 2 سال یا بیشتر یک روش مطلوب برای تغذیه نوزاد است. توانمندسازی شیردهی مادر یکی از عوامل مهم در پیشبرد تغذیه با شیر مادر محسوب می شود و شناسایی عوامل تسهیل کننده آن می تواند به توسعه سیاست های موثر و مداخله کمک کند. هدف از انجام این مطالعه تعیین عوامل مرتبط با توانمند سازی مادران جهت شیردهی در بیمارستان مادر و کودک شیراز در سال 1397 بود. روشها: پژوهش حاضر یک پژوهش توصیفی - کیفی با رویکرد پدیدار شناختی بوده و تعداد مشارکت کننده 12 نفر و از طریق مصاحبه نیمه ساختار یافته صورت گرفته است و تحلیل محتوا همزمان با جمع اوری داده ها صورت گرفت. به منظور تحلیل دادهها در ابتدا هر یک از مصاحبه خوانده شد و مطالب نگارش می گردید. سپس عبارات مهم در محتوای دادهها مشخص و (کدهای باز) آن تعیین شد. بر اساس شباهتها و تفاوتهای موجود و مقایسه مستمر کدهای باز در طبقات مشابهی قرار داده شدند و زیرطبقات را تشکیل دادند و در نهایت درون مایههای اصلی تعیین شد. یافتهها: شرکت کنندگان در پی کسب مهارت های تغذیه با شیر مادر در بیمارستان، مشاوره تغذیه با شیر مادر در مراکز بهداشتی درمانی را به عنوان عوامل مهمی در تسهیل توانمند سازی آنها در شیردهی و تداوم آن عنوان نمودند . تجزیه و تحلیل بیشتر نشان داد "تصمیم مادر برای تغذیه با شیر مادر" همراه با درک او(خودکارآمدی مادر، اعتقاد زن و اعتماد به نفس در توانایی او در تغذیه با شیر مادر) ، "نگرش مثبت فرهنگی و مشارکت همسر و خانواده در تغذیه با شیر مادر"(آموزش و حضور همسر و عشق ورزیدن به مادر در زمان شیردهی )، " آموزش عمومی و ارایه اطلاعات "(یک ارتباط شخصی موفق مادر با دوستانش) و "نقش ماما و سایر کادر درمانی و بهداشتی(تشویق مادران و افزایش ترغیب مادر به شیردهی )" زمینه مناسب را برای تداوم تغذیه با شیر مادر فراهم می کند نتیجه گیری: چهار اصل اصلی از "نقش پرسنل کادر بهداشتی درمانی "، "عوامل فرهنگی " و "عوامل فردی " و عوامل اموزشی از توضیحات شرکت کنندگان استخراج شده است، که نشان دهنده ابعاد تسهیل کننده برای توانمندسازی زنان در شیردهی است. یافته های پژوهش حاکی از آن است که خدمات شیردهی بیمارستانی نقش کلیدی در توانمندی در شیردهی دارد و وجود واحد اختصاصی آموزش و مشاوره شیردهی در بیمارستان، تأکید بر انجام تماس پوست با پوست مادر و نوزاد تأکید بر شروع زودرس شیردهی، حمایت و آموزش عملی مادر و ارجاع موارد مشکل دار به واحد مشاوره شیردهی منجر به شکل گیری توانمندی مادر در شیردهی می شود.

مقدمه: یکی از ابزارهای مهم در برانگیختن پرستاران جهت ارائه خدمات باکیفیت، انگیزش می باشد. پژوهش حاضر با هدف بررسی عوامل کاهنده انگیزش موثر بر عملکرد در پرستاران بیمارستان های آموزشی درمانی دانشگاه علوم پزشکی شیراز انجام پذیرفت. روشها: این مطالعه توصیفی- تحلیلی در سال 1398 در شیراز انجام شده است. جامعه پژوهش تمامی پرستاران بیمارستان های آموزشی دانشگاه علوم پزشکی شیراز بوده که از بین آنها 340 نفر به عنوان حجم نمونه از بین 10 بیمارستان انتخاب گردید. داده ها از طریق پرسشنامه گردآوری و توسط آزمون های آماری t-test، ANOVA و ضریب همبستگی پیرسون در نرم افزار SPSS23تحلیل شد. یافتهها: میانگین کل عوامل مادی و غیرمادی کاهنده انگیزه، به ترتیب برابر با 24/1±14/3 و 31/1±29/3 بدست آمد. در میان عوامل مادی، عامل "مشکلات اقتصادی شخصی" با میانگین 14/1±38/3 و در میان عوامل غیرمادی، عامل "عدم ثبات در تصمیم گیری های مدیران مافوق" با میانگین 13/1±31/4 بیشترین امتیاز را به خود اختصاص دادند. بین متغیرهای جنسیت، وضعیت تاهل، سابقه شیفت، تعداد شیفت در هر ماه، تعداد بیمار تحت نظر در هر شیفت کار با "عوامل مادی" و بین "عوامل غیرمادی" کاهنده انگیزه با متغیرهای سن، سابقه خدمت، و سطح تحصیلات رابطه آماری معنی دار مشاهده شد(p˂0.05). نتیجه گیری: با توجه به یافته ها، برگزاری دوره های آموزشی برای مدیران مافوق در رده های مختلف مدیریتی در زمینه مهم ترین موارد کاهنده انگیزه، فراهم آوردن محیط کاری مناسب توام با تعامل مطلوب و رفع مشکلات اقتصادی پرستاران پیشنهاد می گردد.

پیش بینی خودکارآمدگی شغلی بر اساس تفکر انتقادی و شخصیت پویا در کارکنان دانشکده دندانپزشکی دانشگاه علوم پزشکی شیراز
مقدمه: هدف از انجام این پژوهش پیش بینی خودکارآمدی شغلی بر اساس تفکر انتقادی و شخصیت پویا بود. روش پژوهش توصیفی از نوع همبستگی بود. روشها: جامعه آماری شامل کلیه کارمندان شیف صبح دانشکده دندانپزشکی شیراز بود که از آن میان تعداد ۸۰نفر به روش نمونه گیری در دسترس به عنوان گروه نمونه انتخاب شدند. برای جمعآوری داده ها ازمقیاس خودکارآمدی شغلی (سارجی، 2015)، تفکر انتقادی (ریتکس، 2003) و شخصیت پویا (بایتمن و کرانت، 1994) استفاده شد. به منظور تجزیه و تحلیل دادهها از ضریب همبستگی پیرسون و تحلیل رگرسیون چندگانه با استفاده از نرم افزار SPSS استفاده شد. یافتهها: نتایج نشان داد که بین شخصیت پویا و تفکر انتقادای با خودکارآمدی شغلی رابطه معناداری وجود داشت. همچنین نتایج تحلیل رگرسیون چندگانه نشان داد که شخصیت پویا و تفکر انتقادی به صورت معناداری خودکارآمدی شغلی را در کارکنان پیش بینی می کنند. نتیجه گیری: این نتایج تلویحات مهمی در زمینه آموزش و ارتقای بهداشتی روانی کارکنان در بر دارد.

مؤلفههای مؤثر بر منتورینگ دانشجویی در حوزه آموزش علوم پزشکی-یک مطالعه کیفی
مقدمه: منتورینگ دانشجویی تاثیرگرفته از ابعاد متعددی است که برآیند آنها منجر به تربیت هدفمند و با کیفیت دانشجویان و تحویل فارغ التحصیلان موفق به عرصه جامعه بهویژه در حوزه علوم پزشکی است. مطالعه حاضر با هدف تعیین مؤلفههای مؤثر بر منتورینگ در زمینه آموزش علوم پزشکی انجامگردید. روشها: دادههای این مطالعه کیفی از طریق انجام16 مورد مصاحبه نیمه ساختاریافته با اساتید منتور هیـات علمی دانشگاه علوم پزشکی شیراز به دست آمد. به منظور تجزیه و تحلیل مصاحبه ها از روش تحلیل مضمون استفادهشد. یافتهها: طی این مطالعه،45 مورد مضمون پایه استخراج شد، پس از آن بر اساس کلیت و ترکیب این مضامین، تعداد 19 مضمون سازمان دهنده منتجگردید. در مرحله بعد، 7مضمون فراگیر بهدستآمدکه عبارتبودند از: مؤلفههای انسانی(منتور)، مؤلفههایانسانی(منتی)، مؤلفههایبالینی، مؤلفههای آموزشی، مؤلفههایساختاری، مؤلفههای اجتماعی و مؤلفههای پژوهشی. نتیجه گیری: نتایج به دست آمده از این مطالعه حاکی از آن بودند که مؤلفههای متعددی در پیاده سازی منتورینگ در عرصه آموزش علوم پزشکی دخالت عمده دارند که به ضرورت توجه هر چه بیشتر مسئولین به این مؤلفهها در سطوح مختلف مدیریتی دانشگاه، اشاره و تاکید شد.

امروزه ناباروری یکی از مشکلات شایع برای زوجین محسوب میشود و نیمی از علل آن مربوط به مردان است .حفظ پارامتر های اسپرم و کروماتین آن در آزمایشگاه برای باروری موفقیت آمیزاین زوج ها ازاهمیت خاصی برخوردار است. عوامل متعددی از جمله دما، مدت زمان و روش های مختلف نگهداری اسپرم بر روی پارامتر های اسپرم و کروماتین آن موثر هستند. روش های مختلف نگهداری اسپرم هر یک دارای مزایا و معایبی هستند،از میان این روش ها انجماد شایع ترین روش است که قابلیت بارورسازی اسپرم را در یک دورة زمانی طولانیتر حفظ میکند. اگرچه باروری اسپرم ها در این روش، در مقایسه با مایع جنسی تازه کمتر است اما مزایای این روش از معایب آن بیشتر بوده و هنوز به عنوان ابزار مهمی برای نگهداری اسپرم در کلینیک های ناباروری مطرح است. ما با استفاده از پایگاه داده PubMed به انجام جستجو پرداختیم. درمطالعه ما، ترکیبی از اصطلاحات مرتبط با دما و مدت زمان نگهداری و هم چنین روش های مختلف نگهداری اسپرم و پارامتر های باروری وجود دارد.جستجو با استفاده ازAdvanced Search Builder انجام شد وکلمات کلیدی در "عنوان یا چکیده" یافت شدند. این مطالعه، مروری براثرات دما ، مدت زمان و روش های مختلف نگهداری بر پارامتر های اسپرم و کرماتین آن می باشد تا بتوانیم شرایط و روش های بهینه برای نگهداری اسپرم را دریابیم.
