طراحی الگوی استقرار تخت های مراقبت ویژه در بیمارستان های ایران
مقدمه: تخت های بیمارستانی گرانترین بخش از سرویس ارائه خدمات در نظام سلامت کشورها هستند و در این میان تخت های مراقبت ویژه اهمیت ببیشتری دارتد . این مطالعه با هدف طراحی الگوی استقرار تخت های مراقبت ویژه در کشور انجام شد. مواد و روش: مطالعه حاضر از نوع پژوهش های توصیفی –تحلیلی بوده که در سال 97 انجام شد. جامعه پزوهش شامل 340 نفر از مدیران و گان حوزه سلامت در 10 قطب آمایشی دانشگاههای علوم پزشکی کشور بود. داده ها از طریق پرسشنامه محقق ساخته جمع آوری و با نرم افزارSPSS 25 و Lisrel8.8 تحلیل شد. از تحلیل عامل اکتشافی و تائیدی جه تائید مدل و همچنین از روش تحلیل عناصر اصلی با چرخش عمودی و با تکنیک واریماکس، اجزای اصلی عوامل موثر بر استقرار تخت های مراقبت ویژه تعیین شد. یافته ها: در تحلیل عاملی اکتشافی، 34 متغیر در قالب 6 مؤلفه اصلی تقسیم شدند. نتایج شاخص های برازش برای مدل نشانگر پذیرش شاخص مورد مطالعه و پذیرش مدل است. همچنین 6 عامل اقتصادی (با5 مولفه)، سازمانی (با 7 مولفه)، سیاسی –اجتماعی (با 5 مولفه)، ساختاری (با7 مولفه)، دموگرافیک (با 5 مولفه)، جغرافیایی (با 5 مولفه)، بعنوان ابعاد اصلی شناسایی شدند که 59.36 در صد از کل واریانس متغییرها را تحت پوشش قرار دادهاند. پس از انجام تحلیل عاملی تاییدی، کلیه عوامل در حد مطلوب قرار داشتند و مدل، برازش خوبی رانشان داده و مورد تایید بود. بعد دموگرافیک دارای بیشترین وزن (0.80) و بعد اقتصادی (0.53) دارای کمترین میزان تاثیر بر استقرار تخت های مراقبت ویژه بودند. بحث و نتیجه گیری: در این مطالعه مدل استقرار تخت های مراقبت های ویژه تببین گردید و از میان مولفه های تاثیرگذار، مولفه عامل دموگرافیک نقش بسیار مهمی در توزیع تخت های مراقبت های ویژه دارد. سیاستگزاران و مدیران نظام سلامت جهت نیل به عدالت در سلامت باید در تصمیم گیری برای توزیع مناسب منابع بیمارستانی به خصوص تخت های بیمارستانی به مولفه های موثر برآن توجه ویژه ای داشته باشند.

بررسی مرفومتریک تریون و اهمیت بالینی آن برای اعمال جراحی مغز
مقدمه : تریون درزی در بخش طرفی جمجمه است که در عمق آن عروق مننژ میانی، ریشه شیار طرفی مغز، سینوس کاورنوس، ناحیه بروکا و قشر اینسولا قرار دارد. به علاوه، لند مارکی برای کرانیوتومی و اپروچ های جراحی در تومورهای ناحیه حلقه ویلیس یا لوب فرونتال برای جراحان مغز است. نظر به اهمیت مرفومتری تریون برای نورولوژیست ها، در تحقیق حاضر شاخص های مرفومتریک تریون و انواع آن در جمجمه های خشک بررسی گردید. مواد و روش: مطالعه حاضر از نوع توصیفی و در بازه زمانی 97-96 بر روی 37 جمجمه سالن تشریح که دفرمیتی و مشکل پاتولوژیکی نداشتند انجام گرفت. فاصله عمودی تریون تا مرکز قوس گونه و فاصله افقی آن تا درز فرونتوزیگوماتیک اندازه گیری شد. برای بررسی انواع تریون از تقسیم بندی مورفی استفاده شد. در نهایت، داده ها وارد نرم افزار SPSS شده و با آمار توصیفی و T-test آنالیز شد. یافته ها: در مجموع 37 جمجمه مورد بررسی قرار گرفتند. میانگین فاصله عمودی تریون تا مرکز قوس گونه 41/1± 99/1 سانتیمتر و میانگین فاصله افقی تریون تا درز فرونتوزیگوماتیک 76/0± 80/2 سانتیمتر بود. درصد فراوانی نوع اسفنوپاریتال تریون 1/89%، نوع فرنتوتمپورال4/5%، نوع ستاره ای 7/2% و نوع اپی پتریک 7/2% بود. آزمون آماری تفاوت معنی داری را بین فاصله تریون تا قوس گونه (16/0=p) در سمت چپ و راست نشان نداد. تفاوت معنی داری بین فاصله تریون تا درز فرنتوزیگوماتیک در سمت چپ و راست مشاهده شد (005/0=p). بحث و نتیجه گیری: بررسی خصوصیات مرفولوژیکی استخوان جمجمه برای نورولوژیست ها، متخصصین پزشکی قانونی و آناتومیست ها سودمند می باشد. به هر حال، تحقیقات دیگری با حجم نمونه بالاتر بر روی استخوان جمجمه و نیز کلیشه های رادیوگرافی و سی تی اسکن در نقاط مختلف کشور لازم و ضروری به نظر می رسد.

اپیدمیولوژی و ویژگی های بالینی صرع لوب تمپورال در جنوب ایران
مقدمه: اطلاعات کمی در مورد اپی لپسی لوب تمپورال در کشورهای درحال توسعه وجود دارد. دراین مطالعه سعی شده است که خصوصیات دموگرافیک و علایم بالینی صرع لوب تمپورال در جنوب ایران بررسی شود. مواد و روش: دراین مطالعه مقطعی، تمام بیماران مبتلا به صرع لوب تمپورال که به کلینیک صرع دانشگاه علوم پزشکی شیراز ازسال 2008 تا 2013 مراجعه کردند مورد مطالعه قرارگرفتند. متغیرهای دموگرافیک وعلایم کلینیکی این بیماری شرح داده شده است. یافته ها: 225 بیمار(113 مرد:112 زن) مورد مطالعه قرار گرفتند. سن شروع تشنج 7/17 سال بود. تشنج کمپلکس پارشیال در 179 بیمار (5/79%) گزارش شد. تشنج تونیک- کلونیک در 160 بیمار (1/71%) و اورا در 139 بیمار (7/61%) گزارش شد. امواج فوکال تشنجی در 201 بیمار (6/90%) مشاهده شد. ام ار ای در 39 بیمار (17.3%) نرمال بود و در 117 بیمار (52%) اسکلروز مدیال تمپورال مشاهده شد. بحث و نتیجه گیری: صرع لوب تمپورال یک سندرم اپی لپتیک شایع می باشد. سن شروع بیماری از محدوده وسیع نوزادی تا بزرگسالی متغیر است. تشنج کمپلکس پارشیال شایع ترین نوع تشنج دراین سندرم است .نوار مغز و ام ار ای تست های تشخیصی جانبی هستند. یافته های غیرمعمول در پاراکلینیک در بیماری که شرح حال مشخص صرع لوب تمپورال دارد نباید از تشخیص صحیح جلوگیری کند.

مقدمه: دوران دانشجویی، دوران مهیج و پرچالشی می باشد. در طول این دوره کلیه دانشجوبان باید از سلامت روانی و خود اتکایی بیشتری برخوردار باشند تا بتوانند به توفیق روز افزون در تحصیل و حرفه خود دست یابند. از آنجایی که قسمت عمده ای از موفقیت تحصیلی بستگی به وضعیت روانی دانشجویان دارد، لذا این مطالعه با هدف، بررسی وضعیت روانشناختی و عملکرد تحصیلی دانشجویان پرستاری در دانشکده پرستاری و مامایی شیراز اجرا گردید. مواد و روش: در این مطالعه ی مقطعی وضعیت روانی 144 نفر از دانشجویان پرستاری بر اساس پرسشنامه نشانگان روانی (SCL-90-R) مورد بررسی قرار گرفتند. پس از گردآوری پرسشنامه ها، دانشجویان بر اساس نمراتی که در آن کسب کردند، به دو گروه اصلی فاقد مشکل روانی و دارای مشکل روانی تقسیم شدند. در پایان دوره وضعیت تحصیلی آنها مشخص گردید و میزان رخداد افت تحصیلی در دو گروه تعیین شد. نتایج: آنالیز داده ها نشان داد که 2/42 درصد دارای نشانه هایی از اختلالات روانی بودند، ولی ارتباط آماری معنی داری بین مشخصات دموگرافیک و معدل ترم های تحصیلی (10/2±98/15) با وضعیت روانی افراد مشاهده نشد (05/0> p). هرچند بین وضعیت روانی فرد با میزان علاقه فرد به رشته ارتباط آماری معنی داری وجود داشت (05/0>p). بحث و نتیجه گیری: دانشجویان به دلیل شرایط ویژه ای دارند در معرض اختلالات روانی هستند که می تواند بر وضعیت تحصیلی آنان موثر باشد. لازم است در جهت تأمین و بسترسازی مناسب جهت پیشگیری از این اختلالات در دانشگاهها تدابیر ویژه ای اندیشیده شود.

آیا برنامه های آموزش مداوم موجب بهبود وضعیت تجویز داروها در ایران می شود؟ یک مطالعه کیفی
مقدمه: برگزاری دورههای بازآموزی و آموزش مداوم از جمله استراتژی های توصیه شده جهانی بمنظور دستیابی به تجویز منطقی داروها توسط پزشکان است. مطالعه حاضر با هدف بررسی موانع و پر کردن شکاف بین وضعیت فعلی و وضعیت مطلوب برنامههای بازآموزی، دیدگاه صاحبنظران را در رابطه با نقش این برنامه ها در بهبود وضعیت تجویز داروها ارزیابی نمود. مواد و روش ها: این مطالعه به شیوه کیفی و با استفاده از مصاحبه نیمه ساختاریافته انجام گردید. مشارکت کنندگان مشتمل بر افراد باتجربه و صاحبنظر در زمینه تجویز و مصرف دارو بودند. 16 نفر با روش نمونه گیری هدفمند مورد مصاحبه قرار گرفتند. مصاحبه ها پس از پیاده سازی به روش تحلیل مضمون ارزیابی شدند. یافته ها: در این مطالعه دو مفهوم عمده شامل دلایل نیاز به برگزاری برنامه های آموزش مداوم و موانع برگزاری دوره های آموزش مداوم اثربخش توسط مشارکت کنندگان ارائه گردید. دلایل نیاز به برگزاری برنامه های آموزش مداوم شامل چهار موضوع اصلی و موانع برگزاری آموزش مداوم شامل شش موضوع اصلی (موانع فیزیکی، موانع مالی، موانع انگیزشی، موانع نظارتی، موانع اجرایی و موانع سیاستگذاری) و 12 زیرموضوع بودند. بحث و نتیجه گیری: با توجه به وجود موانع متعدد بر سر راه دستیابی به برنامه های آموزش مداوم تأثیرگذار بر وضعیت تجویز داروها و به منظور رفع این موانع و دستیابی به وضعیت مطلوب، بازنگری در محتوا و کیفیت برگزاری دوره های بازآموزی، برنامه ریزی بر اساس دستاوردهای مطلوب و ارتقای کیفیت مستمر ضرورت دارد.

زنان و طب غیر رایج: مطالعه عوامل مرتبط با استفاده زنان از طب سنتی و مکمل
مقدمه: اقبال رو به افزایش زنان به طب سنتی و مکمل نشاندهندهی تغییراتی است که در حال وقوع میباشد. پژوهش حاضر با هدف مطالعه عوامل مرتبط با میزان مصرف زنان از طب سنتی و مکمل انجام شده است. مواد و روش: این پژوهش مطالعهای تحلیلیمقطعی بوده که در آن461 نفر از زنان مراجعه کننده به ۶ مرکز طب سنتی و مکمل شهر شیراز، با کسب رضایت از آنها و به روش تمامشماری مورد مطالعه قرارگرفتند. دادهها با استفاده ازپرسشنامهی محققساخته که اعتبارو پایایی آن تاییدگردید، جمعآوری و با نرم افزار Spss تحلیل شدند. مدل اجتماعی-رفتاری استفاده از خدمات مراقبت از سلامت آندرسون نیز به عنوان مدل نظری انتخاب گردید. یافته ها: طبق مدل نظری عوامل مرتبط با استفاده از طب سنتی و مکمل در قالب سه دسته عوامل مستعد کننده، عوامل توانمند کننده و نیازها مورد مطالعه قرار گرفتند. نتایج نشان دادند که متغیرهای وضعیت اشتغال، نگرش به طب سنتی و مکمل، شبکه های اجتماعی و دسترسی به طب سنتی و مکمل دارای رابطه مثبت و معنادار با میزان استفاده از طب سنتی و مکمل است. بحث و نتیجه گیری: کلیددستیابی به مهمترین عوامل مرتبط با میزان استفاده زنان ازطب سنتی ومکمل در عوامل مستعدکنندهای چون شبکههای اجتماعی و نگرش مثبت به طب سنتی و مکمل و عوامل توانمندکنندهای مانند وضعیت اشتغال و دسترسی به طب سنتی و مکمل نهفته است. از آنجا که یافتههای پژوهش حاضر حاکی از عدم رابطه بسیاری از عوامل دموگرافیک با میزان استفاده از طب سنتی ومکمل است، بهتر است تمرکزتحقیقات آتی به سایر علل اجتماعی، فرهنگی مرتبط با استفاده از طب سنتی و مکمل باشد.

تبیین عوامل مؤثر در توسعه روابط درون بخشی مدیران بیمارستانهای دانشگاه علوم پزشکی شیراز
مقدمه: مدیران نقش عمدهای در رهبری و سرپرستی منابع انسانی بالینی و اداری جهت بهبود کارایی و اثربخشی در بخشهای مختلف بیمارستان ایفا میکنند. بنابراین لازم است مدیران ارتباطات اثربخشی را برقرار نمایند. این مطالعه با هدف شناسایی مهمترین عوامل مؤثر در توسعه روابط درون بخشی مدیران مراکز درمانی دانشگاه علوم پزشکی شیراز انجام شد. مواد و روش: این پژوهش مطالعهای کیفی است که به روش تحلیل محتوی در سال 1397 انجامشده است. جامعه آماری 8 نفر از مدیران با حداقل 5 سال تجربه مدیریت در دانشگاه علوم پزشکی شیراز بودند که بهصورت نمونهگیری هدفمند انتخاب شدند. برای گردآوری دادهها از مصاحبه نیمه ساختاریافته استفاده شد. درنهایت پس از انجام هشت مصاحبه عمیق و نیمه ساختارمند، یافتهها به حد اشباع رسید و مفاهیم اصلی استخراج گردید. یافتهها: یافتههای تحقیق نشان داد که مهمترین عوامل مؤثر بر توسعه روابط درون بخشی مدیران دانشگاه در سه بعد عوامل فردی با مفاهیم اصلی رهبری، ویژگیهای اخلاقی، ویژگیهای شخصیتی، مهارتهای ارتباطی، بعد عوامل فنی با مفهوم اصلی فنّاوریهای نوین و بعد عوامل سازمانی و ساختاری با مفهوم اصلی فرهنگ سازمانی تقسیمبندی میشوند. بحث و نتیجهگیری: برقراری ارتباطات مؤثر بین بخشهای مختلف یک بیمارستان از وظایف مدیران محسوب میشود. بر اساس نتایج مطالعه توجه به ویژگیهای فردی مدیر، علاوه بر فاکتورهای فنی و فاکتورهای سازمانی بهعنوان مهمترین عناصر تأثیرگذار روابط درون بخشی جهت رسیدن به اهداف سازمانی بسیار ضروری است.

ارزیابی عملکرد وب سایت های دانشگاه های علوم پزشکی ایران در بازتاب طرح تحول سلامت
مقدمه: طرح تحول سلامت در راستای اهداف عالی آن از جمله عدالت در بخش سلامت در حال اجراست. یکی از نکات مهمی که به این هدف کمک می کند و در نقشه تحول نظام سلامت آمده است توجه به قابلیت وب سایت ها و خدمات آن ها است. از این رو در این پژوهش به ارزیابی میزان عملکرد وب سایت های دانشگاه های علوم در راستای اهداف طرح تحول پرداخته است. مواد و روش: این پژوهش که از نوع پیمایشی و توصیفی است برای سنجش عملکرد وب سایت ها با توجه اهدافی که در نقشه تحول سلامت به آن اشاره شده و با الگو گرفتن از فصل فناوری اطلاعات آن چک لیستی تهیه شده. داده های مربوط به تمام 44 دانشگاه در پایان سال 96 مورد بررسی و تحلیل آماری توصیفی توسط نرم افزار SPSS قرار گرفته و امتیاز هر دانشگاه با توجه به میانگین امتیاز کل رعایت شاخص های مربوط به هر سوال مورد ارزیابی قرار گرفته است. بدین ترتیب که بالاترین امتیاز مربوط هر سوال برای هر دانشگاه یک است و کمترین امتیاز صفر و میانگین امتیازات 5/0 است. یافتهها: تنها 11 دانشگاه امتیاز بالای امتیاز میانگین را از 44 دانشگاه دریافت نموده اند. دانشگاه شیراز با کسب 86/0 ؛ تبریز با 76/0 ؛ سبزوار با 74/0 و ارومیه با 70/0 امتیاز به ترتیب در رده های اول تا چهارم را کسب کرده اند. همچنین هیچ هدفی از اهداف یاد شده در دانشگاه های علوم پزشکی به امتیاز میانگین نرسیده اند. بحث و نتیجهگیری: وبسایت های دانشگاه به تکاپو برای برآوردن اهداف طرح تحول به خصوص در مناطق محروم تر و دانشگاه های تیپ دو و تیپ سه شده اند. خدمات رفاهی الکترونیکی از جمله سامانه نوبت دهی و دیگر سامانه ها در این وب سایت ها به وجود آمده اند که این خود سبب تغییر کاربرد وب سایت ها که بیشتر جنبه اطلاع رسانی داشته اند به سمت خدمات رسانی بوده است.

رهیافت های نوین درمان پیری و چروکیدگی پوست با پپتیدهای زیست فعال صناعی
در طول دو دهه گذشته، به دلیل تکامل تکنیک های طراحی و سنتز و در عین حال درک مکانیسم های مولکولی اثر بخشی پپتیدها در بهبود مشکلات پوستی، کاربرد آنها به عنوان عوامل دارو – آرایشی گسترش یافته است. مزایای متعددی برای پپتیدهای زیست فعال از قبیل دسترسی، عملکرد متنوع و انتخابی در فیزیولوژی پوست و فقدان اثرات جانبی ایمنی زا در تجویز موضعی مطرح شده که توجه ویژه متخصصین پوست به استفاده بالقوه از آنها را به عنوان عوامل ضد چین و چروک پوست جلب نموده است. این پپتیدها دارای نقش های مختلف بیولوژیکی نظیر تنظیم سیگنال های ماتریکس خارج سلولی و تحریک سنتز کلاژن می باشند. همچنین، به عنوان پپتیدهای حامل، آنتی اکسیدانت، مهارکننده میانجی گرهای عصبی یا آنزیمی عمل می کنند. اگرچه، موانع متعددی برای کاربرد گسترده آنها در محصولات موضعی ضد پیری وجود دارد: الف) شواهد بالینی برای اثربخشی درمانی هنوز محدود است، ب) نفوذ پوستی به دلیل هیدروفیل بودن، جرم مولی بالا و اتصال به سایر مواد محدود است، و ج) هزینه های زیاد. انتقال پپتید از پوست به طور موثر توسط جذب افزاها، اصلاح شیمیایی پپتید، میکروانکپسیولاسیون و ایجاد تخلخل های ریز در پوست افزایش می یابد. امروزه بیش از سی نوع پپتید به عنوان عامل ضد پیری پوست معرفی شده که بعضا در فراورده های مراقبت از پوست استفاده می شود. این مقاله خلاصه ای از پپتیدهای موضعی با اثربخشی اثبات شده در مطالعات درون تن ارائه می دهد؛ بعلاوه، چالش ها و رویکردهای انتقال پپتید از راه پوست بحث می شود.
