شناسایی خطرات مرتبط با سلامت در آموزشگاه فنی حرفهای زاهدان به روش آنالیز ایمنی شغلی
زمینه و هدف: کارآموزان بهعنوان شاغلین آینده در کارگاههای فنی حرفهای با مجموعهای از ماشینآلات و فعالیتهای پرخطر مواجهه داشته و ماهیت بسیاری از خطرات برای آنها ناشناخته است. این مطالعه باهدف شناسایی خطرات مرتبط با سلامت دریکی از آموزشگاههای فنی حرفهای به روش آنالیز ایمنی شغلی انجام شد.مواد و روش: این مطالعه مقطعی دریکی از آموزشگاههای فنی حرفهای زاهدان در سال 1390 انجام گرفت بدین ترتیب که، 9 کارگاه آموزشی درودگری، تراشکاری، تعمیرات دیزل، تعمیرات ماشینهای سبک، جوشکاری درب و پنجره، جوشکاری لوله، تأسیسات، فرزکاری و صافکاری بر اساس تکرار و اهمیت فعالیتها موردبررسی قرار گرفتند. 53 فعالیت شغلی موجود به210 مرحله مشخص تقسیم گردیده و با استفاده از چکلیست، مشاهده، مصاحبه و سوابق موجود، خطرات مرتبط با سلامت هر مرحله شناساییشده و سطح ریسک آنها با استفاده از ماتریس ریسک MIL-SID882E تعیین گردید. درنهایت اقدامات کنترلی پیشنهادی برای آنها در برگه کار مخصوص این روش، ارائه گردید و از نرمافزار اکسل 2013 برای توصیف دادهها استفاده گردید.یافتهها: پس از تجزیهوتحلیل خطرات به روش آنالیز ایمنی شغلی، 524 خطر شناسایی گردید که در این میان هیچ خطری در سطح غیرقابلقبول شناسایی نشد، 65خطر در سطح نامطلوب، 257 خطر در سطح قابلقبول ولی نیازمند تجدیدنظر و 202خطر قابلقبول بودند. درودگری، تراشکاری و تأسیسات به ترتیب با81، 75 و72 خطر در رأس پرخطرترین فعالیتهای شغلی شناسایی گردیدند.بحث و نتیجهگیری: با توجه به اینکه کارگاههای پرخطر ازجمله درودگری، تراشکاری و تأسیسات در اکثر محیطهای شغلی وجود داشته و حوادث زیادی را به بار میآورند، آموزش حین انجام کار و اقداماتی از قبیل تهیه دستورالعمل، برگزاری کارگاهها و انجام اصلاحات در جهت انجام کار به طریق ایمن بسیار ضروری و مفید هست.

ارزیابی توصیفی بیماران با آسیب قفسه سینه و نیازمند توراکوتومی: مطالعه دو ساله در بیمارستان نمازی شیراز
مقدمه: تروما از جمله دلایل اصلی مرگ و میر در سراسر جهان است. از میان انواع مختلف صدمات، آسیبهای قفسه سینه مسئول اکثریت قریب به اتفاق این مرگ و میرها هستند.مواد و روش: این مطالعه توصیفی مقطعی آن دسته از موارد ترومای قفسه سینه که تحت توراکوتومی قرار گرفتند را شامل شد. دادههای جمع آوری شده از هر بیمار شامل جمعیتی (سن و جنسیت)، نوع آسیب، نوع مداخله جراحی، حجم هموتوراکس، تعداد واحد گلبول قرمز متراکم تزریقی و عوارض مربوطه بود.یافتهها: در مجموع 81 بیمار (میانگین سنی 72/26 سال) بررسی شدند، که از آنها 74 مورد مرد (4/91% ) و 7 مورد زن (6/8 درصد) بودند. شایعترین نوع تروما، آسیب نافذ بود ( 6/71 درصد). رایجترین رویکرد جراحی استفاده شده توراکوتومی قدامی-جانبی سمت چپ بود (6/55 %). میانگین حجم هموتوراکس 53/1686 سیسی و میانگین تعداد واحد گلبول قرمز متراکم تزریقی 74/3 عدد برای هر بیمار بود. متوسط زمان جراحی 25/161 دقیقه بود. حساسترین ارگان برای آسیب، ریه با شیوع آسیب 4/55 درصد بود. متوسط مدت اقامت در بیمارستان 43/8 روز بود. 5 بیمار (2/6درصد) تحت دومین جراحی قرار گرفتند و 15 مورد زنده نماندند (5/18 %).بحث و نتیجهگیری: با توجه به نتایج این پژوهش، برش قدامی-جانبی مورد استفاده ترین رویکرد جراحی در بیماران مبتلا به ترومای قفسه سینه بود.

رواسازی و اعتباریابی پرسشنامه رفتار تغذیهای کودکان
مقدمه: نیاز مبرمی برای شناسایی و کاربرد ابزار تشخیصی که بتواند رفتارهای تغذیهای کودکان را مورد ارزیابی قرار دهد، احساس میشود. پژوهش حاضر با هدف بررسی مقدماتی ویژگیهای روانسـنجی پرسـشنامـه رفتار تغذیهای کودکان در کودکان 1 تا 5 ساله شهر شیراز انجام گرفت.مواد و روش: نمونهای 350 نفره (280دختر و 270پسر) بـه شـیوه در دسترس انتخاب شدند. پرسشنامه رفتار تغذیهای کودکان(Children's Eating Behavior Questionnaire)، پـس از ترجمـه و آمادهسـازی مقـدماتی، توسـط مادران تکمیل گردید. برای بررسی اعتبار، از بازآزمایی و آلفای کرونباخ استفاده گردیـد. روایی سازه نیز با استفاده از تحلیل عاملی اکتشافی به روش مولفههای اصلی احراز گردید.یافتهها: اعتبـار بازآزمـایی با فاصله زمانی دو هفته و مقـدار آلفـا برای کل پرسشنامه به ترتیب برابر با 71/0 و 74/0 بود. نتایج تحلیل عاملی پرسشنامه نیز به استخراج شش مقیاس فرعی «تمایل نسبت به خوردن، تمایل به نوشیدن، کمخوری هیجانی، پرخوری هیجانی، کند خوردن و عدم لذت، تنوعطلبی» انجامید که در مجموع 53 درصد واریانس را تبیین میکردند.بحث و نتیجهگیری: نتایج حاصل از این مطالعه، نشانگر برخورداری پرسشنامه رفتار تغذیهای کودکان از اعتبـار و روایـی مناسـب بـرای سنجش رفتارهای تغذیهای کودکان در جمعیت کودکان 1 تا 6 ساله ایرانی بود. لذا پیشنهاد میشود در موقعیتهای پژوهش آزمایشگاهی و درمانی مورد استفاده قرار گیرد.

مقدمه: برنامه استراتژیک در بیمارستانها به نوعی سرمایهگذاری در راستای بهبود عملکردشان تلقی میشود. با این حال مدیرانی که میخواهند برنامهریزی استراتژیک را جهت دستیابی به موفقیت مورد استفاده قرار دهند، با موانعی بر سر راه اجرای آن مواجه هستند. هدف از انجام این مطالعه تعیین موانع پیادهسازی برنامه استراتژیک از دیدگاه مدیران بیمارستانهای آموزشی درمانی دانشگاه علوم پزشکی شیراز بود.مواد و روش: این پژوهش از نوع مطالعات مقطعی بوده که به صورت توصیفی-تحلیلی در سال 1395 انجام شد. جامعه پژوهش شامل 40نفر از مدیران داخلی، اداری، مالی و حسابداری، و مدیران پرستاری بودند که بصورت سرشماری انتخاب گردیدند. دادهها با استفاده از پرسشنامه محقق ساخته که شامل سوالات مربوط به موانع تاثیرگذار در پیادهسازی استراتژی ها بود، جمعآوری شد. جهت تجزیه و تحلیل داده از آزمون های T-test، ANOVA، ضریب همبستگی پیرسون و آمارههای توصیفی از طریق نرم افزارSPSS23 استفاده شد. یافتهها: نتایج نشان داد که موانـــع پیادهسازی برنامــه استراتژیک به ترتیب مرتبط با عوامل ساختاری(58/6±12/27)، انسانی(43/4 ± 12/20)، فرایندی(99/3±44/16) و نهادی (80/4 ± 85/15) بود. بین متغیرهای دموگرافیک و موانع پیادهسازی برنامه استراتژیک در بیمارستانها ارتباط معنیداری مشاهده نشد(05/0<P). بحث و نتیجهگیری: باتوجه به نتایج، مدیران بیمارستانها بهتر است با شناخت موانع اجرای برنامه استراتژیک، در راستای رفع این موانع برآیند. لذا با توجه به ماهیت پویای فرایندهای مدیریت پیشنهاد میشود دورههای آموزشی متناسب با نیاز و اولویتهای حاصله از پژوهش برگزار گردد.

همبستگی مهارتهای ارتباط بین فردی با نگرش دانشجویان پرستاری نسبت به کار تیمی
مقدمه: ایمنی بیمار و کیفیت مراقبت، به کار تیمی و مهارت ارتباط بین فردی بستگی دارد. بنابراین برخورداری از نگرش مثبت نسبت به کار تیمی نیز ضروری است. با این حال رابطه نگرش نسبت به کار تیمی با مهارتهای بین فردی مشخص نیست. از این رو مطالعه حاضر با هدف تعیین همبستگی مهارتهای ارتباط بین فردی با نگرش نسبت به کار تیمی در دانشجویان پرستاری انجام شد. مواد و روش: این پژوهش مقطعی در سال 1395 انجام شد. نمونه پژوهش شامل 84 دانشجوی پرستاری با میانگین سنی 02/2±90/21 بود که با روش سرشماری انتخاب شدند. ابزارهای گردآوری دادهها «پرسشنامه مهارتهای ارتباطی بین فردی» و «پرسشنامه نگرش نسبت به کار تیمی» بودند. دادهها با استفاده از نرم افزار SPSS نسخه 16 و با آمار توصیفی، ضریب همبستگی پیرسون و اسپیرمن، آزمون t مستقل و من ویتنی تجزیه و تحلیل شدند.نتایج: یافتهها نشان داد بین مهارت ارتباط بین فردی با نمره کل نگرش نسبت به کار تیمی (001/0>P، 42/0=r) و ابعاد رهبری(001/0>P، 46/0=r)، ساختار تیم (003/0=P، 32/0=r) و ارتباط (04/0=P، 23/0=r) رابطه آماری معنیدار وجود داشت (05/0>P). همچنین دانشجویان مرد نمره نگرش نسبت به بعد ارتباط در کار تیمی بالاتری در مقایسه با زنان داشتند (037/0=P).بحث و نتیجهگیری: یافتهها نشان داد که مهارت ارتباط بین فردی با نگرش نسبت به کار تیمی و ابعاد رهبری، ساختار تیم و نگرش نسبت به ارتباط، همبستگی مثبت داشت. بنابراین میتوان به منظور بهبود نگرش نسبت به کار تیمی، تقویت مهارتهای بین فردی را نیز توصیه کرد.

مقایسه اصول اخلاق حرفهای کنفدراسیون جهانی فیزیوتراپی با مبانی فرهنگ اسلامی
مقدمه: در فرهنگ اسلامی، مسئله اخلاق در علوم پزشکی از اهمیت ویژه ای برخوردار است. یکی از زمینه های علوم پزشکی، که در سال های اخیر رشد چشمگیری داشته است، فیزیوتراپی می باشد. این مسئله ضروری به نظر می رسد که اصول اخلاقی رشته فیزیوتراپی به شکلی همخوان با فرهنگ اسلامی، تدوین گردد و در اختیار فیزیوتراپیست ها قرار گیرد. برای دستیابی به این هدف، لازم است یک سری کدهای اخلاقی تعریف شود که قادر باشد ساختار و چارچوب عملکرد فیزیوتراپیست را شکل داده و راهکاری برای مواجهه با چالش های اخلاقی، فراهم نماید.مواد و روش: در این مقاله سعی بر این است که اصول اخلاقی ارائه شده از سوی کنفدراسیون جهانی فیزیوتراپی با اصول اخلاقی و فرهنگی اسلام، مقایسه و معرفی گردد.یافتهها: از تطبیق و مقایسه ی اصول اخلاق حرفه ای کنفدراسیون جهانی فیزیوتراپی با اصول اخلاقی مفاهیم اسلامی بر می آید، نقاط مشترک و قابل وفاقی بین این اصول مشاهده می گردد.بحث و نتیجهگیری: در جهان بینی متافیزیکی به دلیل اینکه اصل، وجود خداوند است، لذا محوریت پروردگار در همه ی ابعاد از جمله انگیزه و نیت رفتار افراد نیز سرایت می کند . در مقابل، در جهان بینی غیر الهی، محوریت به انسان و جهان است؛ لذا این نوع نگاه به حرفه، از جمله فیزیوتراپی، می تواند منجر به نوعی خلاء روابط معنوی، عاطفی و احساسی گردد.
