چکیده

مقدمه: هزینه‌های بهداشت و درمان‌ یکی از چالش‌های مهم نظام‌های سلامت بوده و یکی از موارد مهم در این زمینه حجم زیاد مراقبت‌های بهداشتی ارایه شده نامناسب یا غیرضروری می‌باشند. هدف مطالعه حاضر تعیین مناسبت اندیکاسیون‌های تجویز ام.آر.آی مغز با استفاده از روش appropriateness rand در بیمارستان‌های شیراز بود.مواد و روش: مطالعه حاضر از نوع مطالعات کیفی بود. برای شناسایی اندیکاسیون‌های استاندارد ابتدا گایدلاین‌های موجود مربوط به خدمت تشخیصی ام.آر.آی شناسایی شد. سپس با استفاده از نسخه ترجمه‌شده و معتبر ابزار appraisal of guidelines for evaluation and research (agree)، مناسب‌ترین و معتبرترین گایدلاین که مناسب نظام سلامت کشور بود، انتخاب شد. در راستای تعیین اندیکاسیون‌های نهایی تجویز، از روش دلفی و گروه متمرکز استفاده شد.یافته ها: در مجموع و پس ازنظر کل گروه متخصصین تعداد 34 اندیکاسیون برای ام آر آی مغز تدوین شد که ناهنجاری‌هایی که نیازمند شفاف‌سازی بیشتر برای درمان مستقیم بودند، شامل بدشکلی آرنولد چایری 1و 2، فلج مغزی، ابنورمالیتی حمله‌ای و پیش‌رونده عصبی، سانحه عروقی-مغزی یا سکته، ناهنجاری ژنتیکی، زوال عقل، تاخیر در رشد و تکامل، آنسفالوپاتی، سردرد کودکان، سردرد بزرگ‌سالان، از بین رفتن شنوایی، هماتوما (خون لخته درون بافت)، هیدروسفالوس، آنسفالوپاتی ایسکمیک هایپوکسیک، پروسه عفونت و تورم، تغییرات وضعیت ذهنی به همراه مستندات و شواهد عینی حاصل از آزمایشات، اختلالات حرکتی، مولتیپل اسکلروزیس، اختلالات نوروکوتانوزپ، ناهنجاری نورواندوکرین(ضایعه غده هیپوفیز)، خیز و پرخونی بینایی همراه با افزایش فشار درون جمجه (پاپیل دمیا)، ارزیابی‌های قبل و بعد از جراحی مغز، حمله ناگهانی روانی، وجود مستندات شنوایی‌سنجی در رابطه با از دست رفتن حس شنوایی، سنکوپ، ضربه به مغز، درد عصب سه‌شاخه، ارزیابی تومور خوش خیم یا بدخیم، ضایعات متعدد شناسایی‌شده در تصویربرداری‌های قبلی بدون تایید پاتولوژی بافت، ناهنجاری‌های عروقی، ارزیابی شنت شکمی، سرگیجه، انواع اختلالات بینایی از قبیل کاهش میدان دید، دیپلوپیا و سایر تغییرات بینایی غیرقابل توضیح توسط و آزمایشات بینایی‌سنجیبحث و نتیجه‌گیری: هزینه‌های فراوان بیمارستانی به‌خصوص هزینه آزمایش ام. ار. ای. لزوم به‌کارگیری اندیکاسیون‌های معتبر برای پیشگیری از آزمایشات غیرضروری و پیشگیری از تقاضای القایی را ایجاب می کند. استفاده از این اندیکاسیون‌های هزینه‌های بیمارانو بیمارستان را کاهش می‌دهد.

Background: The cost of healthcare is one of the major challenges in health systems. In this regard, one of the main issues is the large volume of inappropriate or unnecessary care. Therefore, the present study aimed to determine the appropriateness of prescribing indications for using brain MRI using Appropriateness RAND in Shiraz hospitals.
Methods: In this qualitative study, MRI guidelines were identified in order to determine the standard indications. Then, the most appropriate and reliable guideline that was right for the country's health system was selected using the translated version of Appraisal of Guidelines for Evaluation and Research (AGREE). Focus group and Delphi methods were used to determine the final prescribing indications.
Results: The results led to development of 34 indications for brain MRI, including abnormalities detected in other imaging procedures that require more transparency for treatment, types 1 and 2 Arnold Chiari malformation, cerebral palsy, nervous system disorders, progressive neurological disorder, cerebrovascular accidents, stroke, etc.
Conclusion: The high hospital costs, especially MRI, necessitate using a valid indication for preventing unnecessary tests and induced demand. Using these indications results in reduction of hospital and patient costs.
