چکیده

مقدمه: مدیریت بیمارستان در دهه‌های اخیر با چالش‌های متعددی روبرو بوده است. شناسایی و حل این چالش‌ها موجب ارتقای کارایی و اثربخشی فعالیت‌های بیمارستان می‌شود. این مطالعه با هدف شناسایی و تبیین چالش‌های پیش روی مدیریت بیمارستان انجام شد.مواد و روش: مطالعه کیفی حاضر با رویکرد تحلیل محتوا در دانشگاه علوم پزشکی شیراز در سال 1393-1394 انجام شد. جامعه پژوهش شامل مدیران بیمارستان‌های آموزشی و درمانی، مدیران ستادی دانشگاه، اعضای هیئت علمی و دانشجویان تحصیلات تکمیلی دانشگاه علوم پزشکی شیراز بودند که در مجموع 24 نفر از افراد به صورت هدفمند انتخاب شدند. در این مطالعه جهت گردآوری اطلاعات از روش مصاحبه و بحث گروهی متمرکز استفاده شد. همچنین از روش تحلیل محتوا برای تحلیل داده‌های مطالعه استفاده شد.یافته‌ها: در این مطالعه چالش‌های مدیریت بیمارستان در پنچ مضمون (ساختار و تشکیلات دانشگاه، قوانین و دستورالعمل ها، منابع و امکانات، ضعف دوره های آموزشی و سیاست های کلان و بالادستی) تقسیم بندی شد. تغییر فرهنگ و باورها، تغییر ساختار و تشکیلات و آموزش و توانمندسازی به عنوان راهکارهای عبور از چالش ها بیان شد.بحث و نتیجه‌گیری: این مطالعه نشان داد که مدیران بیمارستان‌ها با چالش‌های متعددی که اکثر آنها مشکلات سازمانی و فرایندی هستند روبرو بودند. افزایش ارتباط بیمارستان با سایر مراکز، حمایت مدیران ارشد از مدیران بیمارستان، تقویت فرهنگ مدیرپروری و شفاف نمودن شاخص‌های بیمارستانی از مهم‌ترین عوامل جهت کاهش این چالش‌ها بود.

Investigation of Hospitals Management Challenges: A Qualitative Study in Shiraz Hospitals

چکیده [English]

Background: Hospital management has been faced with several challenges in the recent decades. Identification and elimination of these challenges lead to improvement of effectiveness and efficiency of hospital activities. This study aimed to determine hospital management challenges.
Materials and Methods: This qualitative study using content analysis approach was conducted in Shiraz University of Medical Sciences in 2014-2015. The study participants included educational hospitals’ managers, senior managers, faculty members, and post-graduate students of the University. In this regard, 24 subjects were selected by purposeful sampling. The data were collected using interviews and focus group discussions and were analyzed through content analysis.
Results: The hospital management challenges were categorized into five themes, namely structure of the University, rules and guidelines, resources and facilities, weakness of educational courses, and macro polices. On the other hand, changing the culture and beliefs, restructuring, education, and empowerment were mentioned as strategies for eliminating the challenges.
Conclusion: This study showed that the hospital managers were faced with structural challenges. Yet, these challenges could be decreased by increasing the relationship between hospitals and other centers, senior managers supporting hospital managers, empowerment of managerial culture, and clarification of hospital indices.
