چکیده

مقدمه: کمبود آنزیم گلوکز 6-فسفات دهیدروژناژ (G6PD)، یکی از شایع‌ترین اختلالات آنزیمی گلبول‌های قرمز خون است که بیش از 400 میلیون نفر را در جهان درگیر کرده است. گلبول‌های قرمز با کمبود G6PD، ظرفیت آنتی‌اکسیدانی کمی داشته و نسبت به همولیز ناشی از ترکیبات شیمیایی مانند داروها و مواد خوراکی اکسیدان مستعد می‌کند. بر این اساس، آنتی‌اکسیدان‌ها می‌توانند نقشی حیاتی در پیشگیری از بروز آسیب اکسیداتیو به گلبول‌های قرمز در این بیماران بازی می‌کنند. این مطالعه با هدف بررسی تأثیر عصاره جینکو بیلوبا در پیشگیری از همولیز گلبول‌های قرمز ناشی از تماس با ماده شیمیایی t-BHP در بیماران مبتلا به کمبود این آنزیم انجام شد.
مواد و روش‌ها: 21 بیمار مرد با تشخیص قطعی کمبود شدید آنزیم (G6PD) که همگی حداقل یک اپیزود حمله همولیتیک را تجربه کرده‌اند وارد مطالعه شدند. جهت القا همولیز نمونه خون بیماران در معرض t-BHP، ماده اکسیدان، قرار گرفت. در مرحله بعد غلظت‌های مختلف عصاره جینکو بیلوبا جهت بررسی پیشگیری از بروز همولیز گلبول‌های قرمز مورداستفاده قرار گرفت.
یافته‌ها: نتایج این مطالعه نشان داد که عصاره جینکو بیلوبا موجب کاهش وابسته به غلظت همولیز گلبول‌های قرمز در معرض t-BHP می‌شود. غلظت‌هایµg/m300 وµg/m400 عصاره جینکو بیلوبا منجر به کاهش قابل توجه در همولیز نسبت به گروه کنترل شدند.
بحث و نتیجه‌گیری: با توجه به نتیجه این مطالعه، عصاره جینکو بیلوبا می‌تواند باعث کاهش همولیز ناشی از ترکیبات اکسیدان در بیماران مبتلا به کمبود آنزیم G6PD و مشکلات ناشی از آن شود؛ بنابراین می‌توان به بیمار توصیه کرد که مکمل جینکو را قبل از تماس با مواد اکسیدان مصرف کند.

Background: Glucose-6-phosphate dehydrogenase deficiency (G6PD) is the most common form of enzymopathy of red blood cells that has affected more than 400 million people worldwide. G6PD deficient erythrocytes show decreased levels of antioxidant capacity and they make one susceptible to chemical induced hemolysis such as drugs and oxidant food ingredients. To this end, antioxidants can play a pivotal role in the prevention of oxidative damage to the erythrocyte membrane in such patients. The present study was conducted with the aim of investigating the impact of Ginkgo biloba in preventing t-BHP-induced hemolysis in G6PD deficient patients.
Methods: Twenty-one male G6PD deficient patients all of whom had experienced at least one hemolytic attack were included in the study. To assess the property of Ginkgo biloba extract (EGb) to prevent hemolysis in G6PD deficient erythrocytes, tert-butyl hydroperoxide (t-BHP), an oxidant, was used to induce hemolysis. Different concentrations of EGb (100, 200, 300, 400 µg/ml) were used in this experiment.
Results: Erythrocytes incubated with t-BHP released 30.47±1.61% (mean ± SD) hemoglobin in the supernatant which decreased in the presence of EGb in the concentration dependent manner.  Concentrations of 300 µg/ml and 400 µg/ml EGb showed significant reduction in hemolysis compared to the control group.
Discussion & Conclusion: Based on the results of this study, we propose that EGb may decrease chemical induced hemolysis and the following complications in G6PD deficient patients. Therefore, it may be possible to recommend Ginkgo supplement to these patients before exposure to oxidant agents.
