مقدمه: یکی از روش‌های جراحی موثر و قابل‌پیش‌بینی برای درمان ضایعات داخل استخوانی پریودنتال بازسازی هدایت شده نسجی (GTR) است. هدف از انجام این مطالعه ارزیابی کلینیکی و مقایسه کارایی غشا آمنیون (AM) به همراه Bio-Oss با غشا Bio-Gide به همراه Bio-Oss در درمان رژنراسیون هدایت شده بافتی GTR)) در ضایعات داخل استخوانی پریودنتال بود.مواد و روش: بیماران مبتلا به پریودنتیت مزمن که نیاز به جراحی رژنراتیو پریودنتال داشتند، تحت درمان فاز اول پریودنتال قرار گرفتند. بعد از یک ماه، ده بیمار مبتلا به پریودنتال مزمن و دارای 2 ضایعه داخل استخوانی دو طرفه با عمق رادیوگرافی ≥4 و عمق پروبینگ ≥6 به طور تصادفی در دو گروه غشا آمنیوتیک +Bio-Oss(گروه آزمایش) و Bio-Gide +Bio-Oss(گروه کنترل)قرار گرفتند. پارامترهای کلینیکی (شامل عمق پاکت PPD- عمق ضایعه استخوانی PB- کسب اتصالات کلینیکی CAL و تحلیل لثه REC) در زمان پایه و 6 ماه بعد از جراحی ثبت شدند. داده ها با استفاده از تی تست تجزیه و تحلیل شدند.یافته ها: در گروه آزمایش  PPD ، PB و CAL  قبل و بعد از شش ماه تفاوت معنی‌داری را نشان داد. علاوه بر این تغییرات درREC , PPD, PB, CAL در گروه کنترل معنی‌دار بود. میانگین تغییرات PPD،BP،CAL قبل و بعد از عمل بین دو گروه از نظر آماری، معنی‌دار نبود. اما افزایش در REC از نظر آماری به طور قابل ملاحظه ای در گروه کنترل بیشتر از گروه آزمایش بود(009/0=p).بحث و نتیجه‌گیری: استفاده از دو غشا آمنیوتیک و Bio-Gide همراه با Bio-Oss در هر دو گروه سبب بهبود در پارامترهای کلینیکی گردید. غشا آمنیوتیک منجر به تحلیل لثه قابل توجه آماری نگردید. غشا آمنیوتیک به عنوان یک غشا سدی جدید در درمانهای GTR پیشنهاد می­گردد.

Comparison of Amnion Membrane with Bio-Gide in Conjunction with Bio-Oss in Treatment of Intrabony Periodontal Defects

چکیده [English]

Background: Guided Tissue Regeneration (GTR) is one of the effective and predictable surgical procedures for treatment of periodontal intrabony defects. The present study aimed to clinically evaluate and compare the efficacy of Amnion Membrane (AM) + Bio-Oss and Bio-Gide + Bio-Oss in GTR treatment of intrabony periodontal defects.
Methods: In this study, chronic periodontitis patients who need periodontal regenerative surgery received phase I of periodontal treatment. After 1 month follow up, 10 patients with bilateral intrabony defects and radiographic evidence of intrabony component of ≥ 4 mm and probing depth ≥ 6 mm were randomly assigned into two experimental and control groups. The experimental group was treated by placement of AM + Bio-Oss, while the control group was managed by placement of Bio-Gide + Bio-Oss. Clinical parameters, including Periodontal Pocket Depth (PPD), Probing Bone (PB) level, Clinical Attachment Level (CAL), and gingival recession (REC), were recorded at baseline and 6 months after the surgery. Then, the data were analyzed using T-test.
Results: A significant difference was observed regarding CAL, PB, and PPD after 6 months compared to before the surgery. In addition, significant changes were observed regarding CAL, PB, PPD, and REC in the control group. However, the mean changes in CAL, PB, and PPD before and after the surgery was not significant between the two groups. Yet, a significant increase was found in REC in the control group compared to the experimental group (P=0.009).
Conclusion: Both AM and Bio-Gide in conjunction with Bio-Oss resulted in improvement of the clinical periodontal parameters. According to the results, AM did not induce any significant gingival recession. Thus, AM is recommended as a new barrier membrane in GTR treatment.
