چکیده

مقدمه: اختلال بدشکلی بدن، به‌صورت اشتغال ذهنی با نقص تخیلی یا تحریف مبالغه‌آمیز یک عیب خفیف و جزئی در ظاهر  جسمانی تعریف می‌شود. هدف پژوهش حاضر بررسی رابطه بین اختلال بدشکلی بدن و علائم افسردگی، هراس اجتماعی و وسواس فکری و عملی در بین مراجعه‌کنندگان به کلینیک‌های زیبایی شهر تهران بود. مواد و روش: این پژوهش از نوع همبستگی بود و جامعه آماری این تحقیق کلیه مراجعه‌کنندگان به کلینیک‌های زیبایی در سال 1391 در شهر تهران بود که از بین آنان تعداد 387 نفر از مراجعان به روش نمونه‌گیری چند مرحله‌ای انتخاب شدند، ابزار پژوهش پرسشنامه‌های مقیاس اصلاح شده اختلال وسواس فکری- عملی ییل- بروان برای اختلال بدشکلی بدن، پرسشنامه افسردگی بک، پرسشنامه هراس اجتماعی و پرسشنامه وسواس فکری- عملی مادزلی بودند. تجزیه و تحلیل داده‌ها با نرم افزار spss نسخه 18 و آزمون تحلیل رگرسیون چندگانه به روش گام به گام استفاده شد.یافته‌ها: یافته‌ها نشان داد که بین علائم وسواس فکری- عملی، علائم افسردگی و علائم هراس اجتماعی با اختلال بدشکلی بدن رابطه معنادار آماری وجود داشت (05/0P<). نتایج رگرسیون گام به گام برای پیش‌بینی اختلال بدشکلی بدن نشان داد که در میان مولفه‌های وسواس فقط بازبینی (19/0=B ، 001/0>P) و تردید (12/0=B ، 001/0>P) و در مولفه‌های هراس اجتماعی ترس (32/0=Beta ، 001/0>P) و ترس به همراه مولفه فیزیولوژیکی (36/0=B ، 001/0>P)، همچنین از میان مولفه‌های افسردگی، مولفه شناختی (23/0=B ، 001/0>P) با اختلال بدشکلی بدن ارتباط مثبت و معناداری داشت.بحث و نتیجه‌گیری: علائم هراس اجتماعی به عنوان مهم‌ترین پیش‌بینی کننده‌های اختلال بدشکلی بدن می‌باشد و پس از آن زیر مقیاس شناختی در اختلال افسردگی و زیر مقیاس بازبینی در اختلال وسواس بیشترین سهم را در پیش‌بینی اختلال بدشکلی بدن داشتند. آنچنان که نتایج تایید کرده‌اند، اکثر افراد مراجعه به کلینیک‌های زیبایی دارای اختلال بدشکلی بدن هستند، لذا پیشنهاد می‌شود، در کلینیک‌های زیبایی، مراجعان قبل از اقدام به جراحی مورد ارزیابی روانی قرار گیرند .

The Relationship between Body Dysmorphic Disorder and Symptoms of Depression, Obsessive-Compulsive Disorder, and Social Phobia in Cosmetic Surgery Patients in Tehran

Background: The present study aimed to examine the relationship between depression, social phobia, and obsession-compulsion symptoms and body dysmorphic disorder in cosmetic surgery patients in Tehran.
 Methods: This correlational study was conducted on all the patients referred to cosmetic surgery clinics in Tehran. Among these patients, 384 ones were selected using simple cluster sampling. The study data were collected using modified Yale-Brown obsessive compulsive scale for body dysmorphic disorder, Maudsley obsessive compulsive inventory, Beck depression inventory, and social phobia inventory.  Then, the data were entered into the SPSS statistical software, version 18 and were analyzed using stepwise multiple regression analysis.
Results: The results showed a significant relationship between depression, social phobia, and obsession-compulsion symptoms and body dysmorphic disorder (p=0.05). Besides, the results of stepwise regression analysis for predicting body dysmorphic disorder indicated that checking and doubt among obsessive-compulsive disorder sub-scales (p<0.001, B=0.19; p<0.001, B=0.12), fear among social phobia subscales (p<0.001, B=0.32), and fear with physiological component among depression subscales had significant positive relationships with body dysmorphic disorder.
Conclusions: Social phobia followed by cognitive subscale of depression disorder and checking subscale of obsessive-compulsive disorder were strong predictors of body dysmorphic disorder. The findings have proved that most cosmetic surgery patients are diagnosed with body dysmorphic disorder. Therefore, cosmetic clinics are recommended to evaluate patients psychologically before performing surgeries.
