بررسی مشکلات بیمه درمانی ایثارگران: یک مطالعه کیفی
مقدمه: ارائه مناسب خدمات سلامت برای ایثارگران، نقش بسیار مهمی در حفظ و رضایت این سرمایههای عظیم انسانی و اجتماعی دارد. هدف این پژوهش تعیین مشکلات بیمه درمانی ایثارگران و افراد تحت تکفل آنان در حوزه سلامت بود.مواد و روش: این مطالعه از نوع پژوهش کیفی و با استفاده از تکنیک دلفی در سال 1394 انجام شد. تعداد 30 نفر از نخبگان، مدیران بنیاد شهید و امور ایثارگران مورد مطالعه قرار گرفتند (6 نفر از ستاد بهداشت و درمان، 5 نفر مدیران کل استانی، 14 نفر از معاونان بهداشت و درمان استانها). بعد از جمعآوری اطلاعات به دست آمده، با کدبندی و دستهبندی اطلاعات، اولویت و مشکلات مربوط به بیمه ایثارگران شناسایی شدند. سپس مضامین و مشکلاتِ شناسایی شده بر اساس نظر شرکتکنندگان اولویتبندی شد.یافتهها: با توجه به نتایج روش دلفی، اولویت و مشکلات اصلی مربوط به بیمه درمانی به ترتیب موضوعات سیاست گذاری بیمهها، مشارکت مالی، پوشش بیمهشدگان، مرجع تصمیم گیرنده، سیستم استفاده خدمات، نظارت و ارزیابی و الگوگیری در ارائه خدمات مربوط به سلامتی در ایثارگران بودند.بحث و نتیجهگیری: طراحی و اجرای یک نظام بیمه درون سازمانی و مستقل با نام بیمه سلامت ایثارگران و افراد تحت تکفل که مسئولیت خدمات مرتبط با سلامتی این قشر را در قالب خدمات پیشگیری و درمانی انجام دهد، ضروری به نظر میرسد.

بررسی توانمندی پرستاران بالینی و عوامل مرتبط با آن
مقدمه: برخورداری از توانمندی منجر به ارائه مراقبت بیمارمحور و ایمن شده و در نهایت به ارتقا مراقبت با کیفیت، کمک می کند. مطالعهای با هدف تعیین توانمندی پرستاران بالینی و عوامل مرتبط با آن انجام شد.مواد و روش: مطالعه حاضر، پژوهشی توصیفی-تحلیلی بود که به صورت مقطعی روی 500 نفر از کارکنان پرستاری دانشگاه علوم پزشکی شیراز انجام شد. نمونهگیری به صورت تصادفی طبقهای بود. اطلاعات با استفاده از فهرست توانمندی پرستاران جمعآوری شد. تجزیه و تحلیل داده ها با استفاده از SPSS نسخه 16 صورت گرفت.یافتهها: میانگین توانمندی پرستاران 03/36±42/164 بود. 65% از پرستاران (268 نفر) دارای توانمندی بالا بودند. بین توانمندی پرستاران بالینی با سن، بخش محل کار، سنوات اشتغال به کار پرستاری، وضعیت استخدامی، عضویت در سازمان نظام پرستاری، دانشگاه محل تحصیل و میزان علاقه به رشته پرستاری ارتباط وجود داشت.بحث و نتیجه گیری: یافتهها گویای توانمندی بالای پرستاران در محیطهای بالینی بود. افراد با وضعیت استخدام رسمی و پیمانی و عضو سازمانهای حرفهای توانمندی بالاتری داشتند. همچنین افرادی که در دانشگاه دولتی و سطح بالا تحصیل کرده بودند، توانمندی بالاتری را گزارش کردند. لذا توجه به این عوامل مرتبط می تواند ارتقا توانمندی پرستاران را در پی داشته باشد. انجام مطالعاتی با هدف تبیین فرایند توانمندی پرستاران پیشنهاد می شود.

بررسی تأثیر آموزش توأم مهارتهای مقابلهای و نقاشی بر منبع چند وجهی کنترل سلامت نوجوانان بیسرپرست
مقدمه: مهارت مقابلهای، توانایی شناخت و کنترل احساسات و ویژگیهای خود و ریشه مدیریت هیجانی است. مهارتهای مقابلهای در حکم مهارتی برای منبع چند وجهی کنترل سلامت در نظر گرفته میشود. تأثیر نقاشی نیز بر افزایش مهارتهای مقابلهای در مطالعات متعدد مطرح شده است. هدف مطالعه حاضر بررسی تأثیر آموزش مهارت مقابلهای مدیریت شخصی توأم با نقاشی بر منبع چند وجهی کنترل سلامت نوجوانان بیسرپرست بود.مواد و روش: این مطالعه نیمه تجربی شاهددار تصادفی شده با نمونه 80 نفری از نوجوانان 12 تا 18 سال با گروه آزمون و کنترل انجام گرفت. این 80 نفر نوجوان بیسرپرست به صورت تصادفی در دو گروه کنترل و ازمون قرار گرفتند. گروه آزمون در معرض آموزش مهارت مقابلهای همراه با نقاشی قرار گرفتند. داده ها یک هفته و شش هفته بعد از مداخله با استفاده از ابزار پرسشنامه مهارت مقابلهای و منبع کنترل سلامت درونی، منبع کنترل مربوط به افراد مؤثر و منبع کنترل مربوط به شانس که آلفای کرونباخ همه پرسشنامه ها 85/0 بود جمعآوری شد و توسط نرم افزار SPSS19 مورد تجزیه و تحلیل قرار گرفت.یافتهها: عاملهای مهارت مقابلهای (04/2±55/34)، زیر مقیاسهای منبع کنترل سلامت شامل: منبع کنترل سلامت درونی(75/3±25/64) در گروه مداخله نسبت به گروه کنترل بعد از آموزش افزایش معنیداری پیدا کردند ( 001/0>p) و بر عکس منبع کنترل سلامت مربوط به افراد مؤثر( 25/6±05/42) و منبع کنترل سلامت مربوط به شانس(17/8±66/39)، در گروه مداخله نسبت به گروه کنترل بعد از آموزش کاهش معنیداری پیدا کرد ( 001/0>p).بحث و نتیجه گیری: آموزش مهارت مقابلهای با نقاشی برای ارتقاء منبع کنترل درونی سلامت نوجوانان بی سرپرست مؤثر میباشد.

مقایسه خلق و خوی شیرخواران دارا و فاقد مشکلات تغذیهای
مقدمه: مطالعه عوامل سببساز مشکلات تغذیهای شیرخواران با توجه به شیوع بالا و پیامدهای منفی آن از اهمیت بسیاری برخوردار است. لذا پژوهش حاضر با هدف مقایسه خلق وخوی شیرخوارانی که به روایت مادرانشان دارا و فاقد مشکلات تغذیهای بودند، انجام گرفت.مواد و روش: در این پژوهش علی- مقایسهای، تعداد 50 نفر زوج مادر- کودک از طریق نمونهگیری در دسترس انتخاب شدند. سن شیرخواران بین 6 تا 12 ماه بود. پس از همتاسازی از نظر سن، جنسیت و ترتیب تولد شیرخوار و نیز سن و تحصیلات والدین، نوزادان بر اساس گزارش مادر در مورد وجود یا عدم وجود مشکلات تغذیهای در دو گروه مستقل جایگزین شدند. برای جمعآوری دادهها، پرسشنامه رفتار شیرخوار- تجدید نظر شده، توسط مادر شیرخوار تکمیل شد. دادهها با آزمون تحلیل واریانس چندمتغیری(MANOVA) و با استفاده از نرمافزار20 SPSS-تجزیه و تحلیل گردید.یافتهها: نتایج نشان داد که شیرخواران دارای مشکلات تغذیهای، ترسوتر(02/0P= و 8/5 F=) و غمگینتر (02/0P= و 7/5 F=) از شیرخواران بدون مشکلات تغذیهای بودند. همچنین این گروه در برابر محدودیتها پریشانی بیشتری نشان میدادند(02/0P= و 5/5 F= و در کل از عواطف منفی بیشتری برخوردار بودند(007/0P= و 01/8 F=). در حالیکه شیرخواران بدون مشکلات تغذیهای نمره بیشتری در مولفه لذت سطح بالا کسب کردند (04/0P= و 1/4 F=).بحث و نتیجهگیری: نتایج مطالعه حاضر بیانگر تفاوت خلق و خوی شیرخواران دارا و فاقد مشکلات تغذیهای بود. این نتایج میتواند در برنامههای پیشگیرانه و درمانی مشکلات تغذیهای شیرخواران مورد توجه متخصصان قرار گیرد.

تاثیر تمرینات هوازی و مکمل 25 هیدروکسی ویتامین "د" بر شاخصهای گلایسمیک و مقاومت انسولین در مردان مبتلا به دیابت نوع 2
مقدمه: دﯾﺎﺑﺖ ﯾﮑﯽ از ﻣﻌﻀﻼت ﺳﻼﻣﺘﯽ در ﺗﻤﺎﻣﯽ ﺟﻮاﻣﻊ ﻣﺤﺴﻮب ﻣﯽشود. مداخلات ورزﺷﯽ و مکمل از ﺟﻤﻠﻪ روﯾﮑﺮدﻫﺎی ﺑﻬﺒﻮد وﺿﻌﯿﺖ اﻓﺮاد دﯾﺎﺑﺘﯽ ﺗﻠﻘﯽ ﻣﯽﮔﺮدد. لذا هدف از پژوهش حاضر بررسی اثرات 8 هفته تمرین هوازی با و بدون مکمل 25هیدروکسی ویتامین Dبر قند خون ناشتا، میزان انسولین پلاسما و شاخص مقاومت انسولین بود .مواد و روش: در این تحقیق از نوع نیمه تجربی، با طرح پیش آزمون- پس آزمون از بین مردان 40 تا60 سال مبتلا به دیابت نوع 2 مراجعهکننده به مرکز دیابت بیمارستان عیسی بن مریم اصفهان، تعداد 48نفر به صورت داوطلبانه و در دسترس انتخاب و در 4 گروه تمرینات هوازی (12بیمار)، گروه تمرینات هوازی و مصرف مکمل 25هیدروکسی ویتامین D (12بیمار)، گروه مصرف مکمل 25 هیدروکسی ویتامین D (12بیمار) و گروه کنترل (12بیمار) قرار گرفتند. برنامه تمرینات هوازی به مدت 8 هفته (3جلسه در هفته، هر جلسه 60 تا 75 دقیقه با شدت 85-60 درصد حداکثر ضربان قلب) اعمال شد. گروههای دریافت کننده مکمل نیز مکمل 25هیدروکسی ویتامینD را به مدت 8 هفته با 50000 واحد خوراکی مصرف کردند. میزان قند خون ناشتا، انسولین پلاسما و شاخص مقاومت انسولین قبل و بعد از مداخله در چهار گروه اندازهگیری شد. در نهایت دادهها با استفاده از آزمون تحلیل کواریانس و آزمون تعقیبی LSD مورد تجزیه و تحلیل آماری قرار گرفت.یافتهها: نتایج تحقیق بیانگر تفاوت معنیداری بین میانگین سطح انسولین پلاسما، قند خون ناشتا و شاخص مقاومت انسولین گروه تمرینات هوازی، گروه تمرینات هوازی و مصرف مکمل، گروه مصرف مکمل و گروه کنترل بود (05/0 > P).بحث و نتیجهگیری: مکملسازی ویتامین D به همراه تمرینات هوازی منجر به بهبود میزان قند خون ناشتا، سطح انسولین پلاسما و مقاومت انسولین گردید. بنظر میرسد انجام فعالیتهای ورزشی و مکمل ویتامین D، میتواند به عنوان یک روش درمانی مکمل در بهبود وضعیت بیماران دیابتی مد نظر قرار گیرد.
