Furthermore, the enzyme with the origin of plasmid causes the bacterial resistance to beta-lactam antibiotics. The aim of this study was to determine antibiotic susceptibility patterns and prevalence of CTX-M-type ESBL in E. coli strains isolated from urine samples by phenotypic and genotypic methods. Methods: This descriptive, cross-sectional study of six-month duration (from December to April 2016) was undertaken to investigate patients’ antibiotic susceptibility. To fulfill that purpose, the urine samples of patients referred to seven clinics of the armed forces of the Mazandaran province were collected and antibiotic susceptibility of isolates by disk diffusion method on Mueller-Hinton agar was tested. To do confirmatory tests, the combined disk test method was performed. The test results were compared using standards Clinical and Laboratory standards (CLSIS). Ultimately, after DNA extraction by PCR method, isolates of ESBL positive for the presence of the CTX-M gene were examined and data were analyzed with SPSS software. Results: Of the total 180 patients, 41 isolates of Escherichia coli were separated. From those, 21 isolates (51.21%) had produced ESBL. In PCR, 4(9.75%) of the ESBL positive isolates contained CTX-M gene. The highest resistance to antibiotics was observed for ampicillin (75.6%), cefalotin (58.5%), and cefazolin (60.9%) and the highest amount of sensitivity to antibiotics was seen for imipenem (75.6%), gentamicin (70.7%), and cefotaxime (60.9%). Conclusion: Based on the observed high percentage of resistance to the third-generation-sfalspvryn, it is vitally important to perform precise and accurate antibiogram. Avoiding excessive over-prescription of antibiotics for infections caused by ESBL-producing organism is an inevitable necessity.}, keywords = {Escherichia coli,Drug Resistance Microbial,PCR,CTX-M}, title_fa = {الگوی مقاومت آنتی بیوتیکی و فراوانی ژن بتالاکتامازیCTX-Mدر ایزوله های اشریشیاکلی مولد ESBL در استان مازندران}, abstract_fa = {مقدمه: اشریشیاکلی یکی از عوامل شایع در عفونت انسانی بویژه عفونت دستگاه ادراری می­باشد. اشریشیاکلی‌های تولید کننده بتالاکتاماز های وسیع الطیف (ESBL) دارای  مقاومت به سفالوسپورین می باشد. وجود این جدایه ها احتمال انتقال ژنهای مقاومت سفالوسپورینی و افزایش مقاومت آنتی بیوتیکی در بین سایر سویه ها را افزایش می دهد. لذا هدف از این مطالعه  تعیین الگوهای حساسیت آنتی بیوتیکی و میزان فراوانی بتالاکتامازهای وسیع الطیف تیپ CTX-M در جدایه های اشریشیاکلی به روش فنوتیپی و ژنوتیپی بود.مواد و روش: این مطالعه توصیفی، مقطعی به مدت شش ماه برروی  نمونه­های ادراری مراجعین به 7 درمانگاه از مراکز درمانی نیروهای مسلح استان مازندران طی دوره 6 ماهه در فاصله زمانی آذر 94 تا اردیبهشت 95انجام شد.گونه های اشریشیاکلی جداسازی و با روش‌های بیوشیمیایی تایید شدند. جهت بررسی حساسیت گونه ها ازروشcombined disk test استفاده گردید و نتایج با استانداردCLSI  مقایسه شد.گونه های مقاوم از نظر وجود ژن CTX-M،توسط روش PCR بررسی و داده ها با برنامه نرم افزاری SPSS تجزیه وتحلیل گردید.یافته ها: از تعداد کل180 نمونه بیمار 41 جدایه اشریشیاکلی جدا شد. از این تعداد 21 جدایه (21/51%) مولد بتالاکتامازهای وسیع­الطیف بودند. در تست PCR 4 مورد (75/9 %) از جدایه ESBL واجد ژن CTX-M بودند. در بررسی مقاومت آنتی بیوتیکی بیشترین مقاومت در برابرآنتی بیوتیک­های آمپی­سیلین(6/75 درصد) سفالوتین (5/58 درصد) و سفازولین(9/60 درصد)بود. بیشترین مقدار حساسیت در برابر آنتی بیوتیک­های ایمی­پنم (6/75 درصد) جنتامایسین (7/70 درصد) و سفوتاکسیم (9/60 درصد) بود.بحث و نتیجه گیری: با توجه درصد بالای مقاومت به سفالسپورین­های نسل سوم، انجام دقیق آنتی بیوگرام و پرهیز از تجویز بی­رویه آنتی بیوتیک­ها در عفونت­های ناشی از ارگانیسم های تولیدکننده ESBL یک ضرورت اجتناب ناپذیر است.}, keywords_fa = {اشریشیاکلی,مقاومت دارویی میکروبی,واکنش زنجیره ای پلیمراز,CTX-M}, url = {https://smsj.sums.ac.ir/article_43962.html}, eprint = {https://smsj.sums.ac.ir/article_43962_661bf20976bcb518e15f641e14c58fc0.pdf} }
